Job overview

The Community Staff Nurse is responsible for the delivery of prescribed nursing care to patients with the out of hours district nursing team.  This would include the holistic assessment of patient care needs and the development, implementation and evaluation of evidence based care.  The post holder will be autonomous in their clinical practice while acknowledging their limitations and actively seek advice as necessary.

The successful candidate, will be skilled and creative and will work as an integral member of staff within an Integrated Cluster Network Team. The post holder will  work closely with the wider integrated team and also with GPs and surgery staff,  to support individuals to have good quality emotional and physical well-being in their own homes.

The successful candidate will support other multi-disciplinary colleagues with all aspects of therapeutic care which focus on patient independence.   There would be plenty of opportunity for professional development. 

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
