Are you enthusiastic, well motivated Occupational Therapist who wants to add value and make a difference to the lives of adults experiencing functional issues at home?
We have an exciting opportunity for a Band 6 Occupational Therapist with community experience to join an integrated community network team in Bridgend which consists of social workers, occupational therapists, district nurses and other allied professionals.
The post holder will deliver Occupational Therapy assessment and intervention within a MDT team setting - ensuring the delivery of an evidence-based, occupationally-focussed specialist service within an integrated team.
The Integrated service aims to prevent admission to hospital, promote recovery and self-management strategies, reduce and manage risk effectively and to help prevent hospital admissions.
You will access regular professional supervision with the OT Team lead, and have access to peer supervision and support via the Band 6 Professional Development Forum.
Main duties of the job
You will be supporting individuals in their recovery – working collaboratively with service users to achieve optimum levels of occupational performance and balance in lifestyle management, motivation, interpersonal skills, cognitive skills, self-care, productivity (education, training or employment) and leisure areas.
OT assessment outcomes will inform appropriate intervention plans, identify strengths and adaptive strategies, support recovery, facilitate early discharge, identify post-discharge support needs. reduce hospital admission rates and promote inclusion.
Working as a lone Practioner within this setting, you will deliver an evidence based service, which adheres to local, national and professional standards - supported by the OT Team Lead working within the Adult Inpatient service.
Risk assessment and management are key elements of this role, and you will contribute an occupational perspective to MDT risk management plans.
The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.

Qualifications and knowledge
Essential criteria
- Degree/Diploma in Occupational Therapy
- Registered member of the Health Professional Council
- Knowledge and understanding of factors that can influence prevention of hospital admission: cognitive, physical, psychological and cultural
- Evidence of commitment to CPD
- Post graduate training/ accreditation in fieldwork education
- Knowledge and application of principles of clinical governance and risk management
- Detailed knowledge and understanding of professional ethics/standards and code of conduct, and their application in practice
- Knowledge of graded therapeutic interventions
- evidence of MDT working

- Membership of professional body
- Management qualifications e.g. ILM.
- Knowledge of standardised and non-standardised assessments that can be used with service user group
