The post holder will plan and deliver a specialist health visiting service to families as part of the pre-birth assessment process in Caerphilly Children’s Services.

The post holder will work in close partnership with Social Workers on a daily basis to offer appropriate assessments and support for ante natal parents-to-be and families with the aim of preventing children entering the care system.

• Take responsibility for managing a caseload of individuals/families with complex needs.
• Use negotiation skills to develop individual Care & Support Plans with individuals/families; continually assessing and evaluating care needs.
• Analyse own practice, identify new ways of working and implement change
• Undertake audit of practice, identify excellence and areas of poor performance, and be able to design programmes to meet any required needs identified.

Clinical:

• Act as a specialist to undertake a key role within the service, with responsibility for early identification of health needs, so that the best interests of the child are always prioritised.
• Undertake the clinical duties and the responsibility of the Health Visiting role for the duration of involvement with the individual/family.
• To initiate the Health Visiting records and complete subsequent documentation following all visits i.e. the Primary Birth Visit
• At the end of involvement to arrange a formal handover and joint visit with the named Health Visitor.
• Deliver an intensive version of the Healthy Child Wales Programme and Early Years Core Programme to antenatal women and families with children less than 6 months old.
• With the case responsible Social Worker, review referrals from health and other agencies, plan bespoke care plans, deliver health visiting interventions and refer to other professionals and organisations as appropriate.
• Participate in specific case reviews as required, using own field of expertise to inform and influence decision making.
• Take responsibility for the delivery of high quality, evidence-based care in own practice and the wider service.
• Develop and agree collaborative public health programmes with families.
• Work in partnership with other agencies, disciplines and third sector organisations to develop initiatives that improve health outcomes for children and families.
• Take responsibility for delivering on key public health targets e.g. immunisations, nutrition.
• Participate in public health campaigns within own sphere of competency.
• Comply with all NMC standards and All Wales policies to ensure effective maintenance of records.
