Job overview

An exciting opportunity exists as part of the Health Board's Clinical Futures programme and redesign work within Frailty/Intermediate Care for a Specialty Doctor to join our Community Resource Team based in Caerphilly.  The Frailty Service is a multi-disciplinary service within the Primary Care and Community Services Division, centred on providing people with care and/or treatment close to home, promoting independence via reablement teams.  The aim of the service is to reduce unnecessary hospital admission by providing safe alternate pathways and minimising hospital stay by facilitating early and safe discharge.

The redesign of services for older people means you will be joining the Division at a truly once in a generation era with the development of a 'best in class' Frailty/intermediate care model.

Main duties of the job

All posts are based in the community and flexible working plans afford aspirants the opportunity to participate in the management of community inpatients, ambulatory clinics, community wards and specialty clinics.  Further training can be developed depending on your specific needs.

The opportunity to expand your skills and knowledge of the community through rotation across boroughs is also advocated and encouraged.  All posts have a non-resident on-call element which involves a seven-day working requirement.  This commitment carries an additional session (pro rata'd for part-time appointments).

ABUHB is highly flexible towards career progression, such as obtaining MRCPUK.  We actively support doctors who may wish to pursue the CESR route for specialist registration.  Audit and QI projects will be a requirement of the post and we have close links with Cardiff University should you want to be involved in research.

This position involves regulated activity with adults as defined by the Safeguarding Vulnerable Groups Act 2006 (amended by the Protection of Freedoms Act 2012) and the following checks will be undertaken following any conditional offer: applicants must hold or be eligible to hold full registration and a licence to practice with the GMC.

Detailed job description and main responsibilities

  • To support the Frailty/Intermediate Care team on a day-to-day operational basis and carry a clinical caseload
  • To focus on providing a 'hospital at home' model of care, 7 days a week, 12 hours a day service as part of the Caerphilly Community Resource Team
  • To be responsible for the co-ordination and management of patients in the community and in community hospitals under the supervision of the Consultants and/or in close liaison with General Practitioners
  • To provide medical support for health and social care services that make up the Frailty/Intermediate Care service
  • To work closely with the Consultants and the Directorate Manager to deliver effective and appropriate intermediate care services to residents of the borough
  • To take a key role in discharge planning, co-ordinating approaches with relatives and other agencies
  • To liaise with patients and relatives/carers and members of the multi-disciplinary team to ensure patients receive a high-quality service and are kept informed of progress as appropriate
  • To be responsible for contemporaneous record-keeping, identifying progress and plans in the patient's case notes
  • To participate in virtual ward rounds, facilitating Falls multidisciplinary meetings and Consultant ward rounds
  • To participate in Frailty initiatives, rapid access and co-geriatric clinics
  • To actively use the direct admission pathway to community hospitals to avoid unnecessary acute admissions to emergency departments
  • To complete accurate and timely discharge summaries and progress reports and ensure General Practitioners are informed of any impending discharges, changes of medication and death
  • To assist the Consultants in the development of community, in-patient (step-up/step-down) and outpatient-based services
  • To actively participate in multidisciplinary audit, clinical effectiveness/health gain initiatives, clinical governance, local clinical audit activities and developing research projects, as appropriate
  • To support the pan-Gwent clinical model of the Gwent Frailty programme.
