Job overview

An exciting opportunity has arisen for a fixed term contract until 31st March 2026 to join the Learning Disability Acute Liaison Team in ABUHB.  The role of the Acute/Secondary Liaison Nurse is vital in driving forward ABUHB’s commitment to reducing the health inequalities of people with Learning Disabilities. The work that they do and the impact it has is directly reportable to Welsh Government.  This is a real opportunity to promote equality of care across the ABUHB footprint and you will be joining an experienced, motivated and committed team.  There is scope for this role to be worked alongside a substantive post if the post holder requires additional hours.  

This post is fixed term/secondment  until 31st March 2026 due to meet the needs of the service

Main duties of the job

The Learning Disabilities Acute Health Liaison service offers support to adults who have a diagnosis of learning disabilities who are attending the hospital for outpatient, day patient and inpatient care.

They offer support to inpatient teams caring for adults with Learning Disabilities. 

The team will work alongside the patient to enhance their understanding around aspects of their care.

They work in collaboration with community learning disabilities teams, care providers, families and carers to enhance patient safety, providing support and advice meeting the patient's needs whilst in hospital. 

This also involves support to plan discharge and aftercare. 

They cover every hospital and in Gwent and with Service users aged 18 years or over 

Their aim is to ensure equal access to generic healthcare 

They raise awareness and promote use of Health Profile to ensure patient safety and positive experiences within secondary care 

They provide information around the process of mental capacity and best interest decision making

They facilitate liaison between secondary care and LD professionals 

The team educate secondary care and acute settings around learning disabilities,  health inequalities and promote the Paul Ridd training.
