Job overview
The Symptom Management Service supports people coping with the impact of health conditions in their lives; including, Long Covid, Chronic Fatigue, Fibromyalgia and Chronic Pain. The clinical pathway is focused on working with people to identify and support their needs, in partnership with GP’s, psychologists, specialist teams and community services. The Rehabilitation co-ordinator will play a crucial role in supporting patients with energy limiting condition and chronic pain, working alongside the co-ordinators in the MSK Hub and in partnership with Leisure and NERS.
Main duties of the job
· The Rehab Co-ordinator is a key member of the Symptom Management Service administrative and clinical team.
· The post holder will support the work of the Symptom Management Service pathway in the development of additional supported self-management approaches.
· The post holder will take responsibility for collaborating with Health Board clinical teams, Local Authority leisure and National Exercise on Referral Teams (NERS) to support the rollout self-management approaches to people with both long term and energy limiting conditions.
· Will be responsible for supporting people to access self-management approaches, for example ESCAPE Pain, combined dietetics and physical activity groups
· The post holder will work independently to defined policies and procedures and be responsible for managing own workload.

Essential criteria
- NVQ Level 3 health and fitness qualification PLUS knowledge of specialist functional terms, organisational policies and procedures and knowledge and experience to deal with non-routine issues such as problem solving for an area of work or developing alternative or additional procedures equivalent to Diploma level to include evidence of managing and updating patient information and content
Desirable criteria
- Health or fitness qualification or experience
Experience
Essential criteria
- Previous experience of working directly with the public on health-related behaviour change
- Previous experience in a health and/or social care environment
- Experience of leisure services risk management processes

- Knowledge of NHS clinical information systems in use across the Health Board (CWS, WPAS, Adastra etc)
