Main duties of the job
Complex Care Nurse Assessor Band 6 - ABUHB Community Huntington’s Team
Full Time: 37.5 hours per week.
Are you an innovative practitioner looking to extend your skills? Are you able to work as an autonomous specialist clinician but also as a core Team member?
We currently have a vacancy for a Band 6 Nurse Assessor in our Multi –Disciplinary Community HD Team. The Team provide specialist assessments and interventions to clients and family members affected by HD across the ABUHB catchment area. Working within the community, primarily seeing people in their own homes, working with clients and their families but also working with clients and staff in Nursing Homes, Group Homes, and inpatient facilities as required.
The HD Team is currently based in Mamhilad (covering ABUHB catchment area), and comprises, a Consultant Clinical Psychologist, Physiotherapy team linked to CHC, a Specialist Occupational Therapist, an Advanced Specialist Speech and Language Therapist and administrative support.
It is not a requirement for applicants to have worked extensively with HD clients but experience in long-term conditions and mental health may be helpful.

Detailed job description and main responsibilities
The successful applicants should have:
Recent NHS or Care Home experience within the last 12 months is essential.
A proven track record of being reliable and dependable.
A flexible approach to work would be a great advantage.

Qualifications
Essential criteria
- 1 st level Registered Nurse (RGN / RMN / RNLD) registered with NMC
- ‘Degree level’ education
- Post graduate specialist training in in Community care / nursing or relevant area of nursing or equivalent
- Adult Safeguarding level 1&2
Experience
Essential criteria
- Relevant post basic experience at band 5 or above.
- Experience of undertaking needs-based assessments and care plans
- Undertaking capacity assessments, gaining formal consent and the application of DoLS and the Mental Health Act.
Desirable criteria
- Experience of caring for individuals with Huntington’s Disease
Knowledge
Essential criteria
- Dealing with cases with complex presentations
- Report writing
- Knowledge of Continuing Care legislation and policy
Desirable criteria
- Evidence of Communication training

You must have appropriate UK professional registration.
