The West Norwich Neighbourhood Community Nursing Team are looking for an enthusiastic Band 4 registered nursing assistant or community assistant practitioner to join our friendly, supportive teams of nurses, health care assistants, assistant practitioners and nursing associates. You will be based at Norwich Community Hospital.

The Community Nursing service treats patients in their own homes, including residential homes and supported living complexes. We also provide some services in a clinic setting including our leg ulcer clinic and catheter clinic.

You will demonstrate an understanding of the role of community nursing. NCH&C is committed to the development of future health clinicians. You will live, breathe and promote NCH&C's Behaviour Framework.

Key responsibilities include

  • To assess, prescribe and deliver care packages for patients. To work within the integrated team to facilitate early discharge from hospital.
  • To work within the integrated team to prevent unnecessary admission to hospital.
  • To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.
  • To process referrals though clinical assessment, evaluating the information received to prioritise a response and allocate to the appropriate service.
  • To liaise and communicate with the general public, other health care professionals, and multi-agencies on a daily basis.
  • To provide clinical assessment for incoming nursing referrals.

Norwich Place is working alongside its commissioners and local service providers as part of the Primary Care Network to develop community based services that support patients to remain at home and receive the services they require in an effective and co-ordinated way.

  • RNA ONLY * Nursing Associate Foundation Degree qualification
  • CAP ONLY * Foundation Degree in Health or Social Care or equivalent demonstrable knowledge and skills acquired through training and experience.

Experience
