To assist the registered nurses and junior doctors in the day to day management of West Newton Ward including the assessment, implementation and documentation of patients care from admission to discharge.

To liaise with all aspects of the MDT to ensure greater patient experience and that service provisions are implemented in a timely, effective and efficient manner.

Contribute to effective communication within the multidisciplinary team to ensure the delivery of high standards of holistic care.

Attend board and ward rounds to ensure greater continuity of care between MDT Assist in the

Participate in monitoring a friendly, professional, open environment where the contribution of all team members is valued to facilitate a climate in which staff would wish to work.

Participate in meetings and share information with other members of the team to assist in maintaining an informed workforce.

Ensure communication is carried out in a manner and at a level and pace appropriate to the recipient using interpreters/aids as necessary to achieve desired outcome.

Participate in the appraisal process agreeing objectives and receiving feedback on performance to ensure continuous personal development within the role.

With the nursing staff ensure that clinical practice has patients and their families/carers at its centre and is evidence based, act as the patients advocate in discussions to support patients in making informed decisions regarding their pathway of care.

Experience
