Inclusion Specialist Community Public Health Nurse

Job overview

The Specialist Community Public Health Nurse for Inclusion will support the development of pathways based on specific geographies and communities within Darlington in order to reduce health inequalities and improve health outcomes for vulnerable communities.

 

Main duties of the job

The post will ensure service delivery reflects demographics, existing locality infrastructure and feedback influences service improvement. Alongside other practitioners within the Darlington 0-19 Service, the tSCPHN for Inclusion will be responsible for expanding outreach to those experiencing the greatest inequalities. The 0-19 Service, whilst retaining the delivery of the Healthy Child Programme, will have a specific focus on early intervention and prevention thus supporting a reduction in health inequalities and improved health outcomes.

The post holder will support the delivery of the Healthy Child Programme to children, young people and their families within targeted populations such as:

·       Gypsy Roma and Travelling community.

·       Asylum Seekers, refugees, or vulnerable migrants

·       Ethnic minority groups

·       family members of the armed forces

·       Workless and low-income families

Essential criteria
  • Specialist Community Public Health Degree
  • Experience of working with communities and involving local people in community development / capacity building initiatives
  • Experience of independent working and decision making
  • Registered Nurse on the appropriate part of the NMC Register
Desirable criteria
  • Leadership qualification
  • Experience of line management
  • Experience of working with GRT community
