Urgent Community Response Triage Practitioner

Job overview
The post holder will work closely with the UCR practitioners, Nursing staff, therapy staff and administrators, to provide rapid delivery of treatment and admission avoidance where appropriate. The post holder will provide clinical expertise, by providing assessment, advice, and information, according to clinical guidance, through telephone triage. They will screen and process all incoming telephone referrals, coordinate and case manage the referrals in a timely manner to enable high quality, compassionate care. The post holder will need to utilise professional judgment, critical thinking to determine to accept the referral at point of call, in accordance with guidance, or facilitate onward referral to other professionals as appropriate. The post holder will help to plan, co-ordinate, implement and evaluate care and assist in the coordination of the patient’s journey in order to minimise waits, delays and prevent avoidable admissions.
Main duties of the job
- To be a central contact for the Adult Community Services, using initiative to triage phone calls and emails ensuring appropriate timely action is taken.
- Work closely with the administration team, using judgement and experience to decide which calls and documents can be delegated to them.
- To be willing to undertake the ‘deteriorating patient’ academic unit at level 6/7 depending on current qualifications at university, organised and funded by HDFT.
- Undertake telephone referrals from Ambulance Crews, ED, 111, GP’s, Community and Specialist therapy/nursing, and self-referrals, using clinical reasoning to triage and either accept the referral or facilitate onward referral to other professionals.
- Attend team meetings and act as a representative of UCR within the MDT, and contribute to discussions with GP’s, proactive care, Community Nursing and Social care practitioners.
- Always practice within own scope of competence, recognising limitations of clinical practice, using critical thinking skills.

Working for our organisation
The UCR team provides interventions within 2 hours and over a 48 hour period to prevent hospital admissions. The UCR team will link in with the wider community response teams, including (among others) ARCH (Acute Response and Rehabilitation in the community and hospital setting), CCT (Community Care Team) and specialist community teams Services are provided and managed by Harrogate and Rural Alliance, a partnership between Harrogate and District Foundation Trust and North Yorkshire County Council.

Essential criteria
- • Appropriate professional qualification e.g. RGN, Occupational Therapist, Physiotherapist or Paramedic, with a minimum of 2 years post qualification experience

- • Completed or working towards professional practice educator’s qualification

- • Experience of a broad range of medical and neurological conditions
