Detailed job description and main responsibilities

To assess, plan, implement and evaluate a programme of Cardiac Rehabilitation as per NICE Guidelines and in line with BACPR standards to cardiac patients in primary and secondary care across the Harrogate and Rural District.

To work independently and in conjunction with other members of the team to deliver this high-quality service to patients following cardiac surgery, ACS or PCI through face-to-face consultations and by telephone both in the hospital and in the community.

To provide education to inpatients with heart failure in Harrogate District Hospital and onward referral to the community heart failure nurse team.

Qualifications

Essential criteria

Registered Nurse
Evidence of study at degree level or equivalent
Teaching or mentorship qualification or willingness to work towards

Desirable criteria

Non-Medical Prescribing or willingness to work towards.
Relevant cardiac courses
Intermediate life support

Experience

Essential criteria

Minimum of 3 years post registration experience caring for patients with cardiac conditions in hospital or community.

Desirable criteria

Experience teaching and presenting at meetings
Experience with audit
Knowledge of electronic patient records

As a Senior Assessor, you have the opportunity to focus on delivering direct care. Furthermore, if you have the relevant experience and skills, you can take on an additional responsibility. We limit this to one responsibility to allow for focused time on that topic, rather than having your time stretched too thinly. When taking on an additional responsibility, your case volume is reduced, making time available for this focused time.

For Senior Assessors, additional responsibility options include supporting more complex cases, ensuring quality assurance, providing training, facilitating family engagement, and contributing to pathway development.
