 Due to re-organisation and restructure in adult community services, we have brand new opportunities for band 5 registered nurses. 

Working to a home first agenda, the role is to support service delivery across inpatient, emergency department and community settings.

Our service consists of Health and Social Care colleagues. Consultants, GPs, Advanced Care Practitioners, Urgent community response practitioners, Social Workers, Nursing, Occupational therapists,  Physiotherapists, Discharge Team, rehabilitation assistants and Multi-Disciplinary Assistants all work together to support patient flow. We operate 7 days, 365 days a year to support some of our most vulnerable, older people with frailty.

 You will be based across our 4 community bases in addition to our new purpose-built Hub at Harrogate Hospital.

Main duties of the job

  • To undertake the provision of high standards of nursing care in relation to unscheduled urgent community response, hospital at home and intermediate care in the community.
  • When required inreach into acute services in ED and acute frailty unit and SDEC 
  • To plan, implement and evaluate episodes of care.
  • To plan and organise workload and delegate to other staff when necessary.
  • To undertake nursing assessment, diagnosis and treatment appropriately.
  • To work alongside senior nurses and Matron to support the delivery of nursing intervention
  • To undertake intermediate care  assessments on behalf of the team

Criteria

Essential criteria
  • Registered nurse, registered with NMC
  • Completed post registration preceptorship.
  • Commit to developing skills to be able to assess patients with complex health care needs in the community and develop care plans to meet need.
Desirable criteria
  • Teaching and assessment qualification/ Mentor qualification
  • Community nursing experience
  • Experience of risk management and governance
  • Supervise and assess staff performance
