The Hospice at Home Team is part of the Community Specialist Palliative Care Team across Northamptonshire. It is a county wide service providing holistic nursing care for end-of-life patients in their own home, over a 24-hour period.  We provide nursing care to patients who require symptom management and end of life care at home.  Excellent communication and interpersonal skills are essential, as you will be required to communicate sensitively and compassionately with patients who are approaching the end of their life and their families.

Experience of  working within the community setting and palliative nursing would be ideal and a passion to provide outstanding holistic nursing care is essential.

This 24 hour service requires you to be flexible to a varying shift pattern, this will include early, late and night shifts and includes rotational weekend and bank holiday working. This post requires travel across Northamptonshire travelling to patients home as required.

Prif ddyletswyddau'r swydd

The post holder will have an important part to play in providing holistic care and meeting the specific needs of these patients, their families and those important to them without direct supervision. This will include:

  • Assessment of needs and symptoms and implementation of individualised care. Giving appropriate information to their families and involve them in developing the plan of care and its ongoing evaluation.
  • Administer prescribed medication, as required.
  • Ensure accurate records are maintained.
  • Ensure co-ordinated team approach by use of effective communication within the team and other community services involved.
  • Giving sensitive information and support to those closest to the patient.

We are looking for a nurse who has personal resilience and can manage the care required for patients and their loved ones who are experiencing emotions expected of those in their last short weeks of life.

The post holder will be expected to assist in the learning and development of students and clinicians.
