Are you ready for a role that makes a real difference?
At NHFT, we’re looking for a Clinical Pharmacist to help deliver outstanding care at our Spinneyfields and Recovering Independence Unit (RIBU) inpatient units, which provide accommodation for people requiring health and social care recovery and rehabilitation.
About the Service
Spinneyfields and RIBU provide accommodation and care for individuals requiring health and social care recovery and rehabilitation, delivered by a dedicated multi-disciplinary team.
Admissions typically follow an acute hospital stay or emergency attendance, helping patients regain independence and avoid unnecessary hospital readmissions.

Main duties of the job
Main Duties of the Role
As the Advanced Pharmacist for Elderly Care, Rehabilitation & Frailty, you will:
• Develop and deliver on-site clinical pharmacy services at Spinneyfields and Turn Furlong.
• Lead, evaluate, and improve clinical pharmacy services for patients in these units.
• Conduct medicines reviews to rationalise complex regimes and optimise therapy.
• Provide expert advice on safe and effective use of medicines within the inpatient areas.
• Prescribe and adjust medication (where qualified), within agreed competence, and monitor patient progress.

• A qualified clinical pharmacist with experience in elderly care, rehabilitation, or frailty services.
• Strong clinical knowledge and ability to work autonomously.
• Excellent communication skills and a collaborative approach to working within multi-disciplinary teams.
• A prescribing qualification (desirable but not essential).

Qualifications
Essential criteria
- Registered with GPhC as a Pharmacist
- MPharm degree, or equivalent pharmacy qualification
- Post-graduate diploma in clinical pharmacy (or equivalent level of knowledge and experience)
- Evidence of recent continuing professional development
Desirable criteria
- Member of Royal Pharmaceutical Society
- Independent prescriber
Experience
Essential criteria
- Post registration experience within the NHS setting
- Experience of working as part of a multidisciplinary team
- Experience of planning, implementing, analysing, reporting and changing practice through audit
- Experience of teaching, mentoring and tutoring
- Experience of planning, implementing, analysing, reporting and changing practice through audit
- Experience of data collection, analysis and report writing
Desirable criteria
- Experience of prescribing within a specialty to optimise medicines for specific patient groups
- Management qualification relevant to the NHS
- Experience of developing and implementing medicines management policies and procedures
- Experience of managing staff
