Are you looking for a rewarding and dynamic role where you can apply your knowledge, contribute to patient safety and be part of a multi-disciplinary team?

At NHFT, we’re looking for a Clinical Pharmacist to help lead and deliver outstanding care to our community inpatient beds.

We have two posts available, to cover sites in Northampton and Daventry or Rushden and Wellingborough.

About the Service

NHFT community inpatient services provide accommodation and care for individuals requiring health and social care recovery and rehabilitation, delivered by a dedicated multi-disciplinary team. 
Admissions typically follow an acute hospital stay or emergency attendance, helping patients regain independence and avoid unnecessary hospital readmissions. 

The patients may:

•    Have had a severe illness
•    Have had a decline in their health after a long-term illness
•    Need medical, physical or specialist rehabilitation
•    Have had a stroke or a brain injury
•    Require end-of-life care
•    Be experiencing frailty
•    Be experiencing confusion or delirium

This is a varied role with the successful applicant needing to work independently and within the pharmacy team. Travel between sites will be necessary and you must be able to travel independently to other bases in the Trust across Northamptonshire.

Qualifications

Essential criteria
  • Registered with GPhC as a Pharmacist
  • MPharm degree, or equivalent pharmacy qualification
  • Post-graduate diploma in clinical pharmacy (or equivalent level of knowledge and experience)
  • Evidence of recent continuing professional development
Desirable criteria
  • Member of Royal Pharmaceutical Society
  • Independent prescriber

Experience

Essential criteria
  • Post registration experience within the NHS setting
  • Experience of working as part of a multidisciplinary team
  • Experience of planning, implementing, analysing, reporting and changing practice through audit
  • Experience of teaching, mentoring and tutoring
  • Experience of planning, implementing, analysing, reporting and changing practice through audit
  • Experience of data collection, analysis and report writing
Desirable criteria
  • Experience of prescribing within a specialty to optimise medicines for specific patient groups
  • Management qualification relevant to the NHS
  • Experience of developing and implementing medicines management policies and procedures
  • Experience of managing staff
