Senior Physiotherapist/Pathway Determiner

FIXED TERM CONTRACT - Don't miss out on this fantastic opportunity to join Rushcliffe as a Senior Physiotherapist working in the community. If you want to be part of a forward thinking and innovative service, this is the opportunity you have been looking for.
In your role you will work closely with Occupational Therapists, Assistant practitioners, support staff, clinical assessors ( UCR team), district nurses, specialist nurses and social workers to provide the best possible care to service users in their own homes.
This post also includes paid enhancements for weekends and bank holidays.
Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment.
You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.

An exciting opportunity has arisen to join our developing integrated community services providing clinical triage, rehabilitation, falls management and support timely discharges from our acute Trust partners.
You will work autonomously as well as part of the multi-disciplinary team, liaising closely with health and social care colleagues to coordinate services, promote recovery from illness and enable health, wellbeing and self-care. You will be visiting patients within their own homes using a goal-orientated focus, to maximise patient’s independence, safety and quality of life.
The post will involve cross site working and community visits.
A full UK driving licence and vehicle for business use is essential for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

You will manage your own caseload in the community. As part of the team you will work closely with the rest of the MDT to provide the best possible outcomes for our service users.
You will have a degree/diploma in Physiotherapy and be HCPC registered. You should possess excellent interpersonal and time management skills, be a good communicator and able to prioritise and work well as a member of a team. An induction period and supernumerary support will be given. You will have significant experience of working in the NHS in the UK as a Band 65Physiotherapist and being part NHS healthcare services.
