The Bucks Primary Care Mental Health Hub (PCMHH) is a new and innovative service, which has been set up as part of delivering the Community Mental Health Framework (CMHF) objectives of providing care and support to people in their own communities. The team brings together a range of health care professionals and support staff, to provide care and
treatment to patients in their own communities.

We aim to deliver more proactive and responsive care to reduce
unnecessary hospital admissions, secondary mental health service referrals, or repeat GP appointments wherever possible. PCMHH Teams aim to offer a collaborative approach to deliver care for people with multiple long-term conditions by providing better, local and faster access to services for patients at an earlier point in the care journey. PCMHH Teams offer careful, holistic assessments based on a biopsychosocial care model.

SUN (Service User Network) is a community support group for people with a personality disorder or complex emotional needs. SUN provides regular
community-based peer support groups offering an opportunity for people to share experiences and support one another in a non-judgemental and
understanding environment. SUN sessions are facilitated by Senior clinician and a specialist Peer Support worker who has lived experience of Personality Disorder. 

Prif ddyletswyddau'r swydd

The post holder will be based in the SUN (Service User Network) part of the PCMHH, whilst also delivering some interventions to clients accessing the wider PCMHH.

In this role, you will be primarily working with people with Personality Disorders, so it is essential that you enjoy working with this client group.  The SUN is a very supportive and experienced clinical team based within the Bucks PCMHH.  SUN has proved a popular resource for many people in Bucks and as such, we are recruiting to this post to support our plans to expand and further develop the service. 

You will work closely with the SUN Lead Facilitator and experienced Peer Support Workers to deliver the SUN weekly programme of support groups across the county of Buckinghamshire. These groups are a mixture community-based face to face group and an online group. Weekly clinical supervision and reflective practice is provided by an experienced Consultant Psychologist and is an essential part of the SUN staff's week.  

In the wider PCMHH, you will also assess and provide treatment in the form of groups and some individual interventions.

  • Core mental health qualification in: mental health nursing (RMN NMC registered), or social work (HCPC registered) , or clinical psychology (accredited by BPS), or psychotherapy (UKCP registered), or psychiatry (RCPsych registered), or occupational therapy (HCPC approved) or equivalent
