Job overview
Are you a Nurse, Social Worker or Allied Health Professional passionate about ensuring that those with learning disabilities and autism receive the best quality of care as close to their home and loved ones as possible? If you are, then this could be the perfect job for you.
In response to events Nationally where people with a learning disability and autism have received less than optimal care we developed our out of area and complex case manager role which ensures that any person placed out of area is reviewed regularly to ensure the care and support they receive is of the highest standard and they are placed in hospital for no longer than is required.
You will work with system partners including specialist social workers and mental health colleagues to ensure wherever possible hospital admissions are avoided and care and treatment is delivered within the home setting. This role is essential to support some of our most vulnerable and misunderstood clients.

Main duties of the job
This role is key to ensuring that those with the most complex needs are supported to access specialist mental health hospital care that is appropriate to their needs and is of high quality. You will also ensure that hospital admission will be for no longer than required and work with system partners to reduce likelihood of admissions and where this is not possible to ensure timely discharges
You will work autonomously to support and co-ordinate, where necessary, the case management of individuals with highly complex health and social care needs. You’ll be working to support individuals and their families/carers in accessing the most appropriate care to meet their identified needs.
In this role you’ll be ensuring the case management is delivered with the service user at the heart of the process and that case management issues are dealt with in line with best practice in a timely manner to achieve best outcomes for the individual. You will be making decisions regarding complex Mental Health case management issues in line with the National guidance and with support of senior colleagues where needed

Detailed job description and main responsibilities
The post holder will work within the Reasonable Adjustment Service within the Learning Disabilities Directorate. They will work autonomously to support and co-ordinate, where necessary, the case management of individuals with highly complex health and social care needs. The post holder will be working to support, individuals and their families/carers in accessing the most appropriate care to meet their identified needs.
Responsible for providing day to day case management expertise for Mental Health Act eligible Patients presenting with highly complex needs and circumstances including monitoring of high cost care packages and 1:1 provision.
Define current processes, facilitate discussions and advise colleagues in regard to all matters concerning safeguarding, Court of Protection, mental capacity and best interest decisions.
Ensuring the case management is delivered with the service user at the heart of the process and that case management issues are dealt with in line with best practice in a timely manner to achieve best outcomes for the individual.
Making decisions regarding complex Mental Health case management issues in line with the National guidance and with support of senior colleagues where needed
Ensuring the delivery of a full, high quality NHS Healthcare Service.
The ability to travel independently between sites within the Trust is essential for this role.

- Qualified Health professional in relevant field e.g. Registered Nurse, Occupational therapist or Social Worker.
- Significant post qualification experience of direct patient care involving people with highly complex health needs.
Desirable criteria
- Should have an appreciation of the relationship between the ICB and individual provider and commissioning organisations
Knowledge
Essential criteria
- Significant knowledge of the #Mental Health Act and the Mental Capacity Act and associated guidance/policies.

- Substantial communication skills; able to adapt complex information to be understood by the intended audience and able to articulate care needs and recommendations of health care requirements (both written and verbal)
