Job overview

We are recruiting Clinical Fellows to join our friendly Interventional Psychiatry Service based at the Warneford Hospital Oxford on a 12-month fixed term contract.

This is a unique opportunity for you to gain experience of rapidly acting antidepressants, especially ketamine, associated group psychotherapy, transcranial magnetic stimulation (rTMS) and ECT. The Oxford IPS ketamine clinic has treated over 500 patients with over 10,000 IV doses of ketamine and has a full programme of NHS and self-pay clinics.  

There are many learning opportunities, including support to attend group therapy training, TMS training, and regular advanced psychopharmacology and complex case discussions with academic colleagues.  For the ambitious, energetic and literate there are also opportunities to be involved in and write up research.

We are looking for candidates who have two years’ experience working in an NHS hospital, with at least 4 months full-time in Psychiatry. This role is particularly suitable for those wishing to intercalate a year behind CT3 and CT4 but is open to FY3/4 applicants with relevant experience and aptitudes, and to others at later stages of training.

Main duties of the job

  • Assessment and review for ketamine, TMS, TDCS, ECT treatment
  • Administration of IV ketamine, TMS and ECT
  • Prescribing oral ketamine in liaison with pharmacy technician
  • Teaching, supervising and covering ECT
  • Co-facilitating related psychotherapy groups – relational psychodynamic
  • Sub-investigator on clinical studies

Detailed job description and main responsibilities

The name ‘Interventional Psychiatry Service’ reflects the new modalities of treatment for resistant affective disorders which are provided through what was formally the ECT service. These currently include ketamine, related rapidly acting antidepressants administered by a variety of routes, repetitive transcranial magnetic simulation (rTMS), transcranial direct current stimulation as well as electroconvulsive therapy.
 
The service currently provides:
1. ECT treatment on Tuesday and Friday mornings to between 0-10 patients (median 6)
2. Ketamine treatment to Oxfordshire patients on Tuesday and Fridays (4-8 patients)
3. NHS self-funded ketamine treatment on Mondays, Wednesdays and Thursdays (27 patients a week)
4. Psychotherapy groups for NHS and private patients
5. A rTMS service at the SANE POWIC building
6. Assessments of suitability for rTMS or ketamine treatment
7. Follow-up of patients receiving ECT, rTMS or ketamine
8. A one-stop shop assessment service for potential ECT outpatients
 
The service operates a shared care arrangement with clinical teams, providing treatments in the IPS modalities and some further pharmacological advice. However, overall responsibility for clinical care of patients stays with referrers.
 
The ketamine service is the best-established service for delivering ketamine in the UK and has treated over 400 patients with over 3,000 intravenous doses of ketamine and over 9,000 oral doses. Adjunctive psychotherapy includes a relational psychodynamic group for selected patients in the maintenance phase, and group preparation and integration sessions. The post holder may be involved in either or both of these.
 
The post-holder will participate in developing new aspects of the service, research projects, a monthly peer support group, a fortnightly international journal club and an International Conference. Support for external training (eg in group psychotherapy, TMS) is provided depending on need and interest.
 
The post-holder will work with the IPS staff and be supported and supervised by Professor McShane who leads the service. Responsibilities will be commensurate with competencies as established through external and internal training, including RCPsych ECT training events, GCP and research study specific training, ketamine-related CPD, rTMS training. The post holder will be supervised and directed by the IPS consultant.
