THIS POST IS FIXED TERM  FOR 12 MONTHS DUE TO SECONDMENT COVER.

The Home Treatment Crisis Resolution Team is based at Bronllys Hospital, near Brecon. The team provides Acute Psychiatric Care to individuals at home rather than inpatient care. The service operates between 9:00am and 9:00pm, 7 days a week. The service is provided to the populations of Breconshire, Radnorshire and Ystradgynlais.  The team works closely with the Community Mental Health Teams based in the catchment area and inpatient ward.

The post available is a part-time Band 6 Post fixed term for 12 months

The role includes assessing the suitability of home treatment for individuals presenting with acute mental illness or severe distress, which can be safely managed in the person's home. The practitioner will plan and agree a collaborative care plan with the individual within the framework of the Mental Health Bill in Wales, and will be involved in the provision and evaluation of care provided until recovery is established.

Main duties of the job

To work as a Team member in providing a Crisis Resolution Home Treatment  Service (CR/HT) in Powys. The CR/HT service will provide a service for adults who are experiencing a Mental Health crisis and are at risk of admission to 
Hospital. Intensive Home Treatment will be provided in collaboration with the  service user and in the least restrictive setting, 7 days a week, and 365 days a year as per rota. CR/HT will also facilitate the early discharge of those admitted 
to inpatient units, through intensive but time limited intervention in the community.

Essential criteria
  • Registered Mental Health Professional and Registered with the relevant Professional Body
  • Evidence of continous proffesional develpoment
  • Extensive knowledge of mental health issues
  • Advanced knowledge of mental health legislation, law, and practice
  • Knowledge of research based, and evidence-based interventions
  • Advanced knowledge and application of the Care and Treatment plans
  • Knowledge of the Mental Health Act 1983 for those parts of the Act that directly affect the Community Psychiatric
  • Understanding of clinical governance
Desirable criteria
  • WARRN
  • CBT qualifications/Psycho-Social Interventions/teahing cqualification
  • Evidence of personal development/specialist practitioner award

Experience

Essential criteria
  • Sound understanding of community and multidisciplinary work.
  • Experience of working within a community setting and with Severely Mentally ill clients
  • Evidence of supervising and developing junior staff
  • Abilty to meet travel requirments for the post
  • OR for Annex 21: Willingness and interest in gaining experience working with people who have a mental health disorder in a community setting
