Job overview

An exciting opportunity has arisen to join the Shropshire, Telford & Wrekin Urgent Community Response & Virtual Wards service as a Nurse Associate. The post will form part of a seven day service based within one of the Trust’s locality teams. The service provides patients who are experiencing an acute change in their health a safe and effective alternative to hospital admission, supported by a mixture of face to face visits and digital technology.

We are looking for a Nurse Associate  who shares our passion for providing excellent quality of care and can help deliver a recommended and agreed treatment/care plan to service users in their own home. The post holder will have full access to the Multidisciplinary Team including consultant, GP, Pharmacist, Nurses, Therapists, and Health Care Support Workers.

 

Main duties of the job

This is a service which is at the forefront of the NHS Long Term Plan Agenda,  to develop Urgent Community Response  Services.  As well aiming to improve our local system responsiveness to managing community health crisis response services.  This role offers an ideal opportunity for a Nurse Associate to join the established Multidisciplinary Team and continue to help shape our future.

Working alongside medics, social workers, therapists, and in partnership with GPs, Shropdoc, 111 WMAS, CCC and other healthcare professionals.  Your role will be to ensure the continuation of a seamless service to avoid unnecessary hospital admissions.

The successful applicants will require the appropriate skills and knowledge outlined in the person specification, and be registered with the NMC.  As well as sharing the Trusts vision to provide assessment and delivery of comprehensive, holistic, evidence-based nursing and care support for the management of acutely unwell patients referred to the Shropshire UCR.

  • Demonstrable experience of working as an Nurse Associate within the NHS or other care setting

Training & Qualifications

Essential criteria
  • Registered Nursing Associate on the NMC register
  • Educated to level 5 Diploma of Higher Education and Foundation degree in nursing associate practice / Nurse Associate Apprenticeship Certificate

Knowledge & Skills

Essential criteria
  • Ability to work effectively as a team player under appropriate supervision, and as part of a multi-disciplinary team
  • Excellent interpersonal and communication skills
  • IT skills appropriate to the role
  • Able to recognise own limitations in the context of the nursing associate role
  • Knowledge of when to seek advice and refer to a registered health or care professional
  • Understanding of the scope of the role of the Nursing Associate in context of the team and the organisation, and how the role contributes to service development
