Job overview
We are seeking a motivated Physiotherapist to join our locality therapy team.
The successful candidate will be based from Ludlow Hospital and work across beautiful south west Shropshire as part of our community therapy team.
The successful individual must have an interest and experience in rehabilitation within a community setting and be able to work as an autonomous practitioner.
Our community therapy pathways include supported hospital discharge, urgent and end of life referrals requiring a 24 hour response and routine community therapy.
This is a 7 day service, with weekend working on a rota basis.

Main duties of the job
The post holders main responsibilities will be providing Physiotherapy assessment and rehabilitation to patients who have a wide range of conditions and complex needs. As well as effectively managing your own and the teams caseload, including triage of referrals and regular caseload reviews.
The team works closely with multidisciplinary and multi agency teams, including nurses, GPs, urgent care colleagues and social care.
As a locality therapist you will work closely with community hospital colleagues and provide support to inpatient teams as required, providing continuity to patients and an opportunity to develop a wide range of skills and knowledge.
We have a strong commitment to clinical supervision and continued professional development, offering a supportive and proactive environment within opportunities for supervision, training, peer support and service development, both locally and County wide.
As a band 6, the post holder will have the opportunity to support and supervise junior and non-registered staff.

Qualifications and Training
Essential criteria
- Physiotherapy degree or diploma
- Evidence of specialist post graduate training and education with older adults.
- Evidence of core rotations (MSK, orthopaedics, neurology, respiratory, care of the older adult)
- Current HCPC registration
Experience
Essential criteria
- Significant post graduate experience in a variety of settings with experience in a community and inpatient setting.
- Experience of working with multi-disciplinary and multi agency teams.
Skills and Knowledge
Essential criteria
- Ongoing awareness of recent developments in practice and research including NSF / NICE guidance
- Understanding of clinical governance and risk management and implications for services
- Excellent time management
- Excellent interpersonal and communication skills
Other
Essential criteria
- Able to meet the travel requirements of the post in community
