Shropshire Community Health NHS Trust has made exciting progress with their Virtual Ward, which is continuing to grow, and as a result, we are now ready to add more Pharmacists. This is a fantastic opportunity for Pharmacist Prescribers wishing to hone their skills for managing frailty. Our Clinical Pharmacy Service is well established and provides high-quality care across a variety of specialties. Are you ready to be at the forefront of innovative service development?

In this role, you will be a lead Pharmacist for the provision of comprehensive clinical pharmacy services to the Virtual Wards. You will be embedded in the multidisciplinary team providing care to respiratory, cardiology, and frailty patients in the Virtual Wards. In addition, you will support further development of clinical and pharmaceutical pathways.

We are looking for someone who has extensive clinical pharmacy knowledge and experience, and the ability to work with a multidisciplinary team. You will need to have excellent communication, clinical, and negotiation skills, as well as experience of supervising other staff and providing education and training.

This role offers flexibility, with the potential to work evenings and, occasionally, weekends, making it suitable for individuals seeking a role that fits around other commitments or interests.

Main duties of the job

  • Responsible for the provision and further development of clinical pharmacy services across Shropshire, Telford and Wrekin (STW).
  • In conjunction with the principal pharmacist, attend and actively participate in Medicine Governance meetings and to work collaboratively with the Medicines Management Team to support delivery of the departmental vision and associated strategic objectives.  
  • Be a key member of the multidisciplinary team in relation to virtual ward.
  • Support development of the virtual ward which provides an alternative clinical pathway for respiratory and frailty patients and those with long term conditions.
  • Advise the Principal Pharmacist of any new developments within the Community Services to ensure that the Pharmacy continues to evolve and meets the changing requirements of clinical governance and medicines optimisation.

Qualifications

Essential criteria
  • Masters Degree in Pharmacy.
  • Registered with GPhC.
  • Clinical qualification at postgraduate diploma level or significant relevant clinical experience.
  • Registered as independent prescriber.

Experience

Essential criteria
  • Pharmacist with hospital or primary care experience.
  • Significant post-qualification experience.
  • Be able to illustrate ability to work with other professionals from the multi-disciplinary team.
  • Thorough knowledge of medicines legislation.

Knowledge

Essential criteria
  • In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
  • Excellent interpersonal, influencing and negotiating skill.
  • Have an understanding of the purpose of strategic development and to be able to develop appropriate policies and systems.
  • Excellent written and verbal communication.
  • Able to demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients).
  • Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for acute and long-term conditions.
  • Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to senior staff or heads of service when appropriate.
  • Ability to produce timely and informative high-level reports.
  • Fully computer literate including video conferencing and able to communicate complex information in appropriate for Microsoft office for report writing.
  • Knowledge of pharmacy software.
  • To be able to organise, plan and deliver complex activities on time and manage own workload and co-ordinate the workload of others.
  • To have a high level of reporting and policy writing skills.
  • To have knowledge and understanding of confidential information management.
