Specialist Physiotherapist - Frailty

Job overview

Our highly regarded frailty team at PRH works as an MDT providing admission avoidance services and therapy to prevent deconditioning and maximise independence in specialist areas of ED and short stay wards. 

This is also an exciting time to join the team with the recent opening of the Frailty Same Day Emergency Care unit (SDEC) at both sites. 

We are proud to offer therapy to patients across 7 days a week.  Working in collaboration with our MDT colleagues across the week enables us to deliver physiotherapy specific services when patients need us. As part of this position the successful applicant would be supported to join our weekend rota’s which cover an acute respiratory, orthopaedic and general therapeutic progression patient list.   There will be a need  for extended working hours of the therapy team to mirror the MDT.

This role is for 37.5 hours per week which can be worked flexibly with collaborative discussion on appointment. There may be an opportunity to rotate cross site between the Royal Shrewsbury Hospital and Princess Royal Hospital.

Main duties of the job

The therapy team operates a Home First ethos and works to creatively implement plans to enable people to return to their usual activities of daily living, within their own home environment wherever possible. 

Whilst this role specialise in frailty at the front door services, it is also part of the wider inpatient therapy team. This offers opportunity to access the support and guidance from the wider team, as well as discussion to create integrated patient journeys. 

Main duties include: 

  • To assess, diagnose/interpret and manage own specialist caseload of complex patients as an autonomous practitioner in accordance with Professional Code of Conduct and Health and Care Professions Council Regulations.
  • Engage in own learning to develop self and service
  • Participate or lead on projects to develop services
  • Provide learning opportunities for others including band 5's and students as well as celebrating learning and achievements with the wider therapy teams.    
  • Participation in our respiratory physiotherapy on-call rota
  • To work closely with team leads, clinical leads and senior therapists to ensure a robust structure of training, supervision and appraisal is delivered to all members within the team.
  • To act as a source of Specialist advice and support within the Therapy Centre and across the Trust and the wider health care system.

Qualifications

Essential criteria
  • Degree or equivalent in Physiotherapy
  • HCPC registered

Knowledge

Essential criteria
  • Evidence of CPD detailing clinical experience relevant to Frailty
  • Evidence sound understanding of assessment processes to support admission avoidance and home first principles
Desirable criteria
  • Knowledge of current NHS strategy and plans
  • Evidence of specialist level clinical reasononing

Experience

Essential criteria
  • Evidence of broad range of pre and post qualification experience in the NHS acute or community settings
  • Evidence of Teaching/student experience
  • Previous experience delivering Acute respiratory physiotherapy
  • Previous experience working as part of an NHS respiratory on-call service
Desirable criteria
  • Experience of audit/research
  • Previous experience working within a frailty / front door NHS service
