Job overview
Within our All Age Continuing Care service we undertake assessments for NHS funding and case management in 2 dedicated teams.
This post holder will manage all of the case management functions for individuals on CHC, Joint Funding , Section 117, and FNC in Shropshire Telford and Wrekin ICB locality .
This is currently done by managing 7 individual teams providing case management in the following areas , Mental Health , Learning Disabilities , Care in the persons own home , care in Nursing homes and for those in receipt of a PHB. most of these teams are led by a Band 7 and supported by 1 or 2 Band 6 case managers. However, in the future we may move to locality areas .
Main duties of the job
To have expert subject knowledge in relation to a wide variety of Individual Commissioning functions,
including, but not limited to NHS Continuing Healthcare, NHS Funded Nursing Care, relevant sections
of the Mental Health Act, NHS Act 2006 (as revised), Who Pays and responsible commissioner
guidance. To use this knowledge to advise members of the public, team members and wider staff within
and outside the organisation.

These include GP and primary care services, hospital care, community healthcare and mental health services.
