Job overview

An opportunity has arisen for a skilled, enthusiastic and motivated individual to join our Northeast Shropshire Community Therapy Team. To provide a high quality Physiotherapy service within a multidisciplinary team, delivering a flexible approach to patient care, in a variety of settings throughout the locality, following the patient through their care pathway.

We will work closely with our health and social care partners to give patients more control over their own care and find necessary treatments more readily available. We will support people with multiple health conditions, not just single diseases, and deliver care as locally and conveniently as possible for our patients. We strive to provide a service that is responsive to the needs of the local population as part of a cohesive multidisciplinary team.

Employed by Shropshire Community Health NHS Trust, the post holder will also have future opportunities to undertake 6 month rotations within Shropshire Community Health NHS Trust (SCHT) and across The Shropshire, Telford and Wrekin ICS.   This will allow exposure to a variety of setting increasing your skills and knowledge base and allowing you to explore where your career could take you.

Main duties of the job

To provide a high quality seamless physiotherapy service from Acute or Community Hospital to Community, responsive to the needs of the local population as part of a cohesive multidisciplinary team.

The post holder will work as an autonomous practitioner to perform assessment of patients with diverse presentations. To use clinical reasoning to diagnose/interpret and then develop, deliver and adapt individualised treatment plans for patients within a clinical area

The post holder will be required to delegate appropriate tasks to physiotherapy assistants and will receive feedback from these staff regarding patient’s progress or deviation from expected treatment pathways. They will be responsible for making necessary changes.

Person specification

TRAINING AND QUALIFICATIONS

Essential criteria
  • Degree or equivalent in Physiotherapy
  • Registered with the Health Professions Council.
  • Evidence of Continuing Professional Development including attendance at recent courses relevant to physiotherapy.

EXPERIENCE

Essential criteria
  • Completed broad range of experience including core areas; respiratory, orthopaedics, musculoskeletal and neurology.
  • Experience of working as part of a physiotherapy team.
  • Experience of working in NHS or similar setting
  • Experience of assistance in student supervision or willing to develop skills

KNOWLEDGE AND SKILLS

Essential criteria
  • Organisation, planning, prioritisation and decision making skills
  • Ability to work under pressure and meet deadlines
  • Understand the legal responsibilities of the profession.
  • Competent in the use of IT
  • Good listening skills
