This is a new role working across a variety of health and community settings to develop a new taskforce to support the pressures of winter. We are working together to support our Somerset citizens to live as independently as possible and remain well wherever they are

We have a number of opportunities for people who have the right values, behaviours and the passion to care to make a difference in people’s lives.

From working with people in hospital who have severe dementia to keep them safe, to supporting someone to leave hospital to a community reablement setting, this role is exciting, rewarding and diverse.   You will need to be mobile and able to work in numerous places across Somerset and working a shift pattern (e.g. early mornings/late evenings) will be required

Main duties of the job

Experience of working in a care setting or supporting someone with their care needs would be advantageous but we will offer all the training you need to help you develop the skills and confidence to support you in your career journey

Individuals will be taken on substantively, but will need to engage with us to look at what roles they will move to following the winter period.  We are looking to develop a new role that will break new ground in the delivery of health and social care in the future and challenge the notion of Health or social care as a career.

Detailed job description and main responsibilities

  • The post holder will carry out well-defined routine duties in accordance with the individual’s in-patient or reablement plan or long term care plan, promote independence and check on the individual’s overall progress, comfort and wellbeing. This may be in any number of care settings, including acute, community and own homes
  • When working as a lone worker the post holder will escalate risks and take measures to make a situation safe in accordance with training provided and report any concerns as required.
  • Work within a range of healthcare settings this will include colleagues from both health and social care.
  • When supporting a reablement care plan, the post holder will report to the key worker responsible for the client who will directly or indirectly supervise their work. On an acute ward it will be the Nurse in Charge
  • To motivate and encourage clients with a diverse range of social needs and medical conditions to achieve agreed outcomes.
  • The post holder will deliver outcome focused support plans and provide a range of care and support tasks to support individuals living within their own homes.

Person specification

Qualifications

Essential criteria
  • Good standard of literacy and numeracy and spoken English (Minimum Level 2, or Functional Skills).
  • Vocational Qualification Level 3 equivalent level or equivalent experience.
  • Level 3 qualification or Certificate in Healthcare, Care Certificate or willing to work within agreed time scale.

Knowledge

Essential criteria
  • Understanding of actions which can improve an individual’s quality of care.
  • The provision and promotion of holistic person-centred care.
Desirable criteria
  • A good understanding of health and social care models.
  • An understanding of the physiology, psychology and organisation and function of the human body/ Basic Medical conditions
