Job Purpose:
· To provide high quality clinical assessment, intervention and treatment for presenting patients
· Demonstrate safe clinical decision making and expert care including assessment and diagnostic skills for patients
· Work collaboratively with the clinical team to meet the needs of the patients, supporting the delivery of policy and procedures
Main duties of the job
· Ensure confidentiality is preserved at all times whether the information is written/electronic or oral and whether the employee is at or away from work
· Work under regulatory ruling of Health and Safety to ensure the control of potential hazards including handling of pathology specimens, personal hygiene, cross infection
· Follow Data Protection and practice protocols for Clinical Governance Framework and Caldicott Principles
· Take all appropriate precautions to protect the security and safety of oneself and others and the premises
· Assess, analyse, diagnose, plan, implement and evaluate highly specialist and complex treatment/interventions and programmes of care for patients presenting with an undifferentiated problem, some of whom will have complex needs and/or high levels of vulnerability
· Where appropriate, triage or stream patients to the most appropriate care pathway
· Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly, advising patients and carers
· Diagnose and treat both acute and chronic conditions, integrating both drug and non-drug based treatment methods into a management plan
· Proactively identify, diagnose and manage treatment plans for patients with long-term conditions or at risk of developing a long-term condition

Detailed job description and main responsibilities
· Recognise and work within own competence and professional code of conduct as regulated by the NMC. Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, practice policies and procedures
· Prioritise, organise and manage own workload in a manner that maintains quality but also recognises overall practice requirements and workload
· Deliver care according to NSF, NICE guidelines, practice protocols and evidence-based care.
· Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation
· Initiate and participate in the maintenance of quality governance systems and processes
· Utilise the audit cycle as a means of evaluating the quality of work of self and nursing team, developing improvements where required
· Evaluate patients’ response to health care provision and the effectiveness of care
· Support and participate in shared learning across the practice
· Use a structured framework to manage, review and identify learning from patient complaints, clinical incidents and near-miss events
· Assess the impact of policy implementation on care delivery
· Help develop the performance of the nursing team in accordance with local policies
· Understand and apply legal issues that support the identification of vulnerable and abused children and adults, and be aware of statutory child/vulnerable patients health procedures and local guidance
· Maintain appropriate skills and knowledge regarding domestic violence, vulnerable adults, substance abuse and addictive behaviour. Be aware of, and use relevant guidance and support to ensure appropriate referral if required

Qualifications/Knowledge
Essential criteria
- Nurse Practitioner Qualification
- First Degree or equivalent advanced clinical skills qualification
- Independent Prescriber
- RGN with current NMC registration
- Knowledge of primary & community health care
- Advanced clinical practice & examination skills
- Ability to form management plan for patients with acute & chronic conditions & complex needs
Desirable criteria
- Masters degree in Advanced
- Clinical Practice Local & national health policy and impact on General Practice
Experience
Essential criteria
- Several years post registration
- Autonomous practitioner with several years recent primary and community nursing experience or in “first presentation setting”
- Nurse-led management of minor illness, minor ailments and injuries and chronic disease management
- Nurse-led triage
- Prevention & screening skills such as cytology
- Qualification in delivering contraceptive services
- Evidence of CPD in autonomous practice
- Ability to evaluate safety & effectiveness of own practice
Desirable criteria
- Involvement in mentoring/teaching learners
- Experience in leadership role & change management
- Experience telephone triage & visiting patient in own home/residential setting
- Good understanding of General Practice Sector
