Job overview

An opportunity for a temporary post/ secondment to the Well-being team has arisen.  The Community Wellbeing Team (SMI Health Checks) provide a health and social care service for people experiencing/recovering from severe and enduring mental health problems. 

The post holder will undertake an annual Physical Health Check as detailed in the QOF guidance for people with a severe mental illness (SMI). They will support people to develop and achieve the outcomes of their Health Action plan.

Main duties of the job

  • To undertake an annual Physical Health Check as detailed in the QOF guidance for people with a severe mental illness (SMI) and ensure all interventions are recorded in the Trust Electronic Patient Record (EPR)
  •       To support people to develop and achieve the outcomes of their Health Action plan, working with other agencies to ensure the highest standard of patient care. 
  •      To be accountable for delivering appropriate and effective evidence-based care to meet the needs of the individual.
  •      To apply motivational interviewing techniques when care planning health interventions.
  •       To work collaboratively with patients and their carers to facilitate and promote rehabilitation and recovery, helping patients to achieve and maintain their optimum level of mental health functioning and quality of life in the community.
  • Provide clinical guidance and support for all members of the multi-disciplinary team

Qualifications

Essential criteria
  • Degree, Diploma or equivalent in R.M.N, and significant pre-qualification experience plus additional training/short courses
Desirable criteria
  • Continuing post qualification Mental health training

Experience

Essential criteria
  • Appropriate experience either pre or post qualification in the field of mental health
  • Management of complex needs
Desirable criteria
  • Work in multi-disciplinary setting.
  • Mental Health experience of Care coordination.
  • Experience of working with mental health illnesses

Knowledge

Essential criteria
  • Knowledge and understanding of current mental health issues.
  • Knowledge of the Mental Health Act and Mental Capacity Act and Care Act including other relevant legislation
Desirable criteria
  • Experience in delivering psycho-social interventions.

Skills

Essential criteria
  • Assessment and organisational skills.
