Job overview

As part of the Rotherham Healthwave team, our Health and Wellbeing Coaches support people from the local community to improve how they manage their physical and, in turn, their mental health.  By working with people both on a one-to-one or in a group setting, you will help people develop the knowledge, skills, and confidence to change their health-related behaviours, helping to improve their overall quality of life, health, and wellbeing. These behaviours can include dietary advice, smoking cessation, and physical activity. As a Health and Wellbeing Coach, you will also be responsible for maintaining up-to-date and accurate clinical records. The role requires coaching skills, patience, and empathy to support people in reaching their goals.  A person-centred approach is critical to the service's success, and although the role can sometimes be challenging, it is also very rewarding.

Working for our organisation

Connnnect Healthcare Rotherham Community Interest Company (CIC) is a federation of 28 GP practices in Rotherham. The organisation is led by a Board of representatives from general practice in the Borough, with executive leadership from a Medical Director on a part-time basis.

The organisation currently delivers extended access primary care services in Rotherham and a Physio First service in partnership with the local acute and community trust, the largest of its kind currently operating. In addition, the organisation is supporting the development of primary care networks and manages delivery of training and development to large cohorts of the general practice workforce.

Detailed job description and main responsibilities

Manage and maintain a caseload of clients throughout their chosen lifestyle intervention. 

Develop individual, evidence-based, tailored support plans based on assessed needs. These plans will include individual and group-based interventions, signposting, and referrals to other health, community, and voluntary services.

Work with clients to set SMART goals using evidence-based knowledge, NICE, and NHS guidelines.

When delivering nutrition/weight management sessions, you will be responsible for taking weights and waist measurements in a compassionate and confidential manner for clients who wish to monitor their progress.

Contribute to the service's monitoring and evaluation, including completing the required data set. This will include following up with clients who do not engage with support and identifying cohorts with multiple risk factors and those most at risk.

Ensure that patient-related documentation is entered into electronic health records with high accuracy and completeness.

Work with managers and IT support to maintain data quality and improve data collection where necessary.

Assist managers and clinicians in achieving contracted targets by ensuring data is entered accurately and promptly.

To manage one's own time and priorities when providing service.

To undertake any other reasonable tasks determined by managers and Connect team colleagues.

To ensure that the confidentiality of staff, patients, and carers is maintained in line with the organisation's policy.

To support safe and effective team practices.

To work closely with team members to provide a flexible service and be willing to assist other staff as and when required.

Attend community and promotional events to talk about the service as required.

Effectively communicate work capacity and pressures within the team and ensure the smooth running of administrative provision. Raising with the line manager any barriers to safe and effective working.

To attend all mandatory training courses required and comply with health and safety regulations as part of personal development, e.g., systems training, fire awareness training, Personal Development Reviews (PDR/Appraisals)

Responsible for the designing/ uploading and staffing rotas for the Weight Management and Nutrition team.

Work as part of the wider Team to offer support in effectively delivering the Rotherham Healthwave

 

Person specification

Essential

Essential criteria
  • Educated to GCSE (or equivalent) grade C or above in English and Maths.
  • Working knowledge of Microsoft Office.
  • Experience in managing highly confidential and sensitive information.
  • Caring and compassionate nature, with the ability to demonstrate sensitivity, empathy, and compassion to service users.
  • Ability to work as part of a team
  • Able to work under limited supervision but willing to access support and escalate issues, as necessary.
  • Ability to travel between sites, with own transport and hold a full UKlicence
Desirable criteria
  • A basic understanding of common mental health problems
  • Knowledge of behaviour change models and experience of using motivational interviewing techniques.
  • Experience providing help, support, and/or counselling on a one-to-one or group basis
  • Working knowledge of electronic health records systems, particularly using SystmOne
  • Qualification in health and wellbeing or health coaching
  • Knowledge of principles of health promotion and issues of health inequalities.
