Working for our organisation
Connnnect Healthcare Rotherham Community Interest Company (CIC) is a federation of 28 GP practices in Rotherham. The organisation is led by a Board of representatives from general practice in the Borough, with executive leadership from a Medical Director on a part-time basis.
The organisation currently delivers extended access primary care services in Rotherham and a Physio First service in partnership with the local acute and community trust, the largest of its kind currently operating. In addition, the organisation is supporting the development of primary care networks and manages delivery of training and development to large cohorts of the general practice workforce.

We are a GP-led organisation and aim to provide services for direct patient care and to support GP practices. We will be a patient-centred, working collaboratively across general practice and with partners to provide high-quality services, positive working experiences for our staff, and to secure the sustainability of general practice in Rotherham

Person specification
Qualifications
Essential criteria
- GCSE Maths and English to at least Grade C, or equivalent.
- Venepuncture certification
- Knowledge on NHS Health Checks
- Qualified in vital observations
Desirable criteria
- Recognised Health Trainer qualifications e.g. NHS Health checks or equivalent, or Understanding Health Improvement or equivalent
Experience
Essential criteria
- Experience in providing health-promoting initiatives/services in the local community.
- Experience in working at events
- Experience in working in community and GP settings.
- Knowledge in completing NHS Health checks.
- A good knowledge of the Rotherham area.
- You will need to be a people-person
- An enthusiast for how healthy lifestyles can impact positively on peoples health
- Someone who is fantastic at building rapport with an ability to persuade
Knowledge
Essential criteria
- Good knowledge of challenges to achieving health improvement's amongst vulnerable and deprived communities.
- Good knowledge of application of behavioural change theoretical principles, to facilitate, motivate and empower individuals / local communities to make substantive and sustainable behaviour change
- Training can be considered for the right candidate.
- Knowledge and certificate on NHS Health checks
Special Conditions
Essential criteria
- Full UK Driving license with access to a car with business travel insurance for work
