Health Coach - Weight and Nutrition Management

Job overview
As part of the Rotherham Healthwave team, our Health and Wellbeing Coaches support people from the local community to improve how they manage their physical, and in turn their mental health. Through working with people both on a one to one or in a group setting, you will help people to develop the knowledge, skills, and confidence to change their health-related behaviours, helping to improve their overall quality of life, health, and wellbeing. These behaviours can include dietary advice, smoking cessation, and physical activity. As a Health and Wellbeing Coach you will also be responsible for maintaining up to date and accurate clinical records. The role requires coaching skills, patience, and empathy to support people reaching their goals. A person-centred approach is key to the success of the service, and although the role can sometimes be challenging it is also very rewarding.

Manage and maintain a caseload of clients throughout their chosen lifestyle intervention. Develop individual, evidence-based, tailored support plans based on assessed needs. This will include individual and group-based interventions, signposting and referring to other health, community, and voluntary services.
Working for our organisation
Connnnect Healthcare Rotherham Community Interest Company (CIC) is a federation of 28 GP practices in Rotherham. The organisation is led by a Board of representatives from general practice in the Borough, with executive leadership from a Medical Director on a part-time basis.
The organisation currently delivers extended access primary care services in Rotherham and a Physio First service in partnership with the local acute and community trust, the largest of its kind currently operating. In addition, the organisation is supporting the development of primary care networks and manages delivery of training and development to large cohorts of the general practice workforce.

Detailed job description and main responsibilities
1. Manage and maintain a caseload of clients throughout their chosen lifestyle intervention.
2. Develop individual, evidence based tailored support plans based on assessed needs. This will include individual and group-based interventions, signposting and referring to other health, community, and voluntary services.1. Work with clients to set SMART goals using evidence-based knowledge, and NICE and NHS guidelines.
2. When delivering nutrition/weight management sessions you will be responsible for taking weights and waist measurements in a compassionate and confidential manner, for those clients that wish to monitor their progress.
3. To contribute to the monitoring and evaluation of the service, including the completion of the required data set. This will include following up clients who do not engage with support and identifying cohorts with multiple risk factors and most at risk.
4. Ensure that patient related documentation is entered into electronic health records with high degree of accuracy and completeness.
5. Work with managers and with IT support to maintain data quality and make improvements to data collection where necessary.
6. Assist managers and clinicians in achieving contracted targets by ensuring data is entered accurately and in a timely manner.
7. To manage own time and priorities in providing service.
8. To undertake any other reasonable tasks as determined by managers and Connect team colleagues.
9. To ensure confidentiality of staff, patients and carers is maintained in line with organisations policy.
10. To support safe and effective team practices.
11. To work closely with team members to provide a flexible service and be willing to assist other staff as and when required.
12. Attend community and promotional events to talk about the service as required.
13. To communicate effectively regarding work capacity and pressures within the team and ensure the smooth running of administrative provision. To raise with the line manager any barriers to safe and effective working.
14. To attend all mandatory training courses required and comply with health and safety regulations as part of personal development, e.g., systems training, fire awareness training, Personal Development Reviews (PDR/Appraisals)
15. Responsible for the designing/ uploading and staffing of rotas for the Weight Management and Nutrition team.
16. Work as part of the wider Team to offer support in effectively delivering the Rotherham Healthwave

Qualifications
Essential criteria
- Educated to GCSE or equivalent grade C or above in English and Math
- NVQ level 3 in Health care or equivalent experience
- Level 3 Diet and Nutrition RQF or equivalent NVQ or experience
Desirable criteria
- Qualification in health and wellbeing or health coaching.
- Working knowledge of electronic health records systems, particularly using SystmOne

- Ability to motivate others.
- Excellent communication skills and ability to maintain professional manner even when under pressure.

Experience
Essential criteria
- Experience of working in the NHS in the community or hospital setting.
- Experience of providing healthcare, healthcare advice or health promotion advice to patients or members of the public
- Ability to deal with the public in an empathic and effective manner
- Ability to liaise with all levels of professionals in a confident and effective manner.
- Experience of managing highly confidential and sensitive information
- Ability to work and communicate in group settings.
- Excellent communication written and verbal and listening skills.
Desirable criteria
- Experience of providing help, support, and/or counselling on a one-to-one or group basis.
- Knowledge and experience of working with primary care-based health professionals.
- Knowledge of behavior change models and experience of using motivational interviewing techniques.
