Job overview

To provide appropriate physical and mental health assessment and intervention for those patients who are on an extended recovery journey with secondary mental health services.

 

To provide a treatment, including depot administration, blood taking and monitoring service to all patients in Rotherham who are currently prescribed Clozapine and Lithium therapies.

 

There is an expectation that the post-holder will work collaboratively with the multi -professional team, to meet the needs of patients.

Main duties of the job

·         Assessment of need and risk.

·         Working in clozapine clinic.

·         Planning and delivering care, including administration of depot medication.

·         Physical Health Monitoring in alignment with best practice for SMI including ECGs, venepuncture and physical health checks.

·         Liaising within the multi-disciplinary care team, including carers, voluntary agencies etc.

·         To provide information as required.

·         To maintain accurate, contemporaneous, accessible records according to both NMC and Trust record keeping standards.

·         To liaise with GPs, primary care and all relevant services across the trust.

Essential criteria
  • Registered Nurse Associate
  • To be a member of a professional body and to abide by their code of conduct and RDaSH trust policies and procedures.

Experience

Essential criteria
  • Care and assessment of people with mental health problems; ability to deal with non-routine and unpredictable nature of workload and individual patient contact.

Knowledge

Essential criteria
  • Interventions, treatment approaches and evidence-based practice in the care of people with mental illness.
  • Knowledge of when to seek advice and escalate to the appropriate professional for expert help and advice
  • Knowledge of appropriate and relevant frameworks, legislation and policy
