Job overview
Band 6 Hospital Liaison Practitioner within the Adult Mental Health Hospital Liaison Team.
Location - Woodlands Unit, Rotherham Foundation Hospital Trust Site
We are seeking an enthusiastic and experienced Mental Health Practitioner to join our Hospital Liaison Team.
Your role will be to provide Urgent and Emergency response to Mental Health Referrals from The Rotherham Foundation Trust (TRFT) with whom we work in close partnership. You will undertake full bio-psychosocial assessments and contribute to providing safe and supported discharge plans. You will work closely with TRFT staff to develop their knowledge of Mental Health- including involvement in delivering training and supporting TRFT staff to develop positive working practices which put the patient at the centre of their own care and promote parity of esteem.
Main duties of the job
You will be a knowledgeable and a competent practitioner with excellent assessment and risk assessment skills, the ability to work well under pressure and demonstrate the confidence and ability to work autonomously and as a member of the wider MDT. You will have experience of working with adults with complex mental health needs and have a willingness to continue your professional development.
The team works over a 24/7 period, and this involves a flexible duty rota covering this period. Full-time applications will only be considered.

Knowledge
Essential criteria
- Ability to use initiative and work without immediate supervision
- Ability to demonstrate an understanding, empathic approach
- Skills in co-ordinating and planning interagency approaches in mental health care
- An understanding of current mental health issues
Qualifications
Essential criteria
- RMN or Registered Social Worker
- Evidence of post registration training
- ENB 998 or other approved mentor qualification or mentor qualification appropriate to discipline - or willingness to complete.
Experience
Essential criteria
- Demonstrable post registration experience as an RMN
- Professional experience of working in acute adult mental health
- Experience of MDT working
- Skills in coordinating and planning inter-agency approaches in mental health care
- Experience of assessing and managing risk
Skills
Desirable criteria
- Excellent report/assessment writing skills
- Good IT skills
- Ability to work as part of a multi-professional team
- Understanding of the CPA process
- Excellent assessment skills, including risk assessment
- Understanding of the importance of user/carer involvement in care planning
- Experience and understanding of Safeguarding responsibilities
