An exciting opportunity to work as a Band 6 Dementia Practitioner joining the STH Dementia Practitioner team and support the Trust in providing a dementia friendly service.
Our aim is to support the Trust to be able to provide a dementia friendly service for people affected by dementia, whether attending STH as a visitor, outpatient or inpatient.
You will help to achieve this by ensuring the physical environment, stimulation through available activities, care provided, information available and sign posting for ongoing support is designed, implemented and coordinated to meet the needs of people with dementia.
Your work aims to enable people with their carers to feel safe, well cared for, valued and their experience is enhanced, and for staff, to feel that they have the appropriate skill set, resources and support to provide a dementia friendly service.
This is a versatile role for a motivated, dynamic person to join the team on a secondment basis for 8 months. The role will end no later than 1st March 2025.

Will have extensive knowledge and proficiency in dementia and delirium, acting as a role model for the organisation.
Will work closely with the Band 7 Lead Dementia Practitioner.
Will provide education for ward staff in dementia and delirium.
Will provide clinical input into the planning, development, and evaluation of STH services.
Will support the ward teams in the care of patients and collaborate with other specialist dementia services locally and nationally.
Will be part of the team leading the development of dementia pathways of care. Share dementia information, providing clinical guidance of practice, involvement in clinical audit, teaching and supporting professional colleagues and ensuring the provision of dementia role modelling.

The STH Dementia practitioner service consists of a team of 4 with varied professional backgrounds. Led by a band 7 Dementia practitioner.
The dementia practitioner team is funded by the Sheffield Hospitals Charity; you will work closely with the STH dementia care group, Sheffield strategy group, and different groups both across the Trust and city.
This is a great opportunity to develop and consolidate a wide variety of skills, and make a real difference and positive contribution to the care of people with dementia.

This vacancy is advertised on a secondment contract basis for a period until 1st March 2025. The secondment contract status of the post holder will be reviewed in accordance with the Trust's legal obligations regarding fixed term contracts and statutory maternity provisions.

- Professional registration
- To have completed the national PEAP programme for practice supervisors or equivalent teaching and assessing qualification for those with ENB998.
- Have completed training in dementia

- Recognised qualification in Dementia or equivalent experience
Experience

- Substantial experience at AFC band 5 (or equivalent)
- Experience caring for people with Dementia and Delirium
