Do the have the skills and passion for working with Children and Young People with a Learning Disability in this supportive, friendly team. This is a Community Learning Disability Nurse post. The job involves working independently with children and young people with a Learning Disability and their families within agreed guidelines, completing assessments (including risk management) and developing evidence-based treatment and/or intervention plans to facilitate positive outcomes in relation to referred behavioural and/or mental health needs.

The post holder will also be required to offer specialist knowledge and skills on a range of behavioural and mental health issues relating to children and young people with a Learning Disability and offer liaison, consultation, learning and development as appropriate.

It is an exciting time to come and work in Shropshire, Telford and Wrekin's Children, Young People and Families Services. We have received additional investment to improve the support available. As a result, we are pleased to be able to advertise a number of new roles both substantive and fixed-term posts. We can welcome clinicians who want to work in Children’s Mental Health Services. Where people are interested in applying but would need to relocate we can support with associated incentives. Where people are interested and passionate about working with children but have identified additional training, we may be able to train staff to develop the appropriate skills. 

Prif ddyletswyddau'r swydd

  • Take responsibility for the management of a complex caseload, which includes, specialist assessment (within service specification), care planning in conjunction with service user and carer, evaluation of individual treatment plans for CYP with learning disabilities.
  • Assess patient's individual care needs, develop, implement and evaluate programmes of care.
  • To work as part of the Children’s Community Learning Disability Team assessing the health and emotional wellbeing needs of children and young people with moderate to severe learning disabilities and responding appropriately.
  • To complete specialist nursing assessment, development, implementation and evaluation of interventions to promote the health and wellbeing of children, young people and families affected by learning disabilities and or complex physical and mental health needs.
  • To work as an integral member of the multi disability team maintaining responsibility for a designated caseload acting with autonomy as appropriate.
  • To positively represent the needs and rights of children and young people with learning disabilities, promoting equality and access to NHS and any other service.

QUALIFICATIONS & TRAINING

  • Registered learning Disabilities Nurse (RNLD) or Mental Health Nurse (RMN),
  • BSc degree in nursing or equivalent level
  • Post-registration experience, evidence of formal courses and training at diploma/degree level.
  • Conforms to NMC requirements

  • Leadership qualification
  • Qualification in providing supervision
  • Qualification in coaching
  • Psychological intervention Qualification including EMDR, CBT, CBTp, Family Interventions, CAT, DBT
  • Registered nurse mentor/supervisor

EXPERIENCE

  • Evidence of extensive post registration experience with working with children & young people with learning disabilities
  • Experience of working with people who have difficulty in engaging with services and have complex mental health needs
  • Experience of working in community setting and making decisions autonomously
  • Experience of managing health needs related to medication optimisation
  • Experience of assessing, planning, co-ordinating and managing people with mental health problems in a community setting
  • Experience of integrating care delivery across multi-disciplinary /multi agency teams
  • Experience of leading/contributing to CPA
  • Experience of dealing with mental health related issues/referrals/need for advice by telephone including at times of high demand and in relation to highly stressful/emotive situations.
