Job overview
To support transition into primary and secondary care provision in MPFT or local area secondary mental health or other appropriate services. The post holder will be responsible for undertaking clinical assessments, in order to establish the person’s needs, goals, risks and strengths in order to generate a formulation based care plan agreed with all involved.
They will be responsible for delivering nursing interventions, in accordance with an agreed care plan, to an identified group of people experiencing mental health related difficulties.
Main duties of the job
To assist in the assessment, planning, implementing and evaluating of outcome focused, evidence based interventions for individuals within the IOM programme
o To work in partnership with a caseload of service-users under multi-agency IOM management, who are experiencing complex Mental Health needs.
o To promote service-users’ recovery and to signpost to services or third sector organisations that will support them in regaining and maintaining their place in the community

Detailed job description and main responsibilities
Working within a multi-agency integrated offender management team, covering contractually agreed hours per week, with the flexibility of working across the locality of Telford & Shrewsbury.
o Assessment of/ and offering support in crisis situations, if necessary referring to the Crisis/ home treatment team.
o Assessment of individuals, selected to the IOM scheme, for any mental health, needs.
o Provision of effective short term interventions and management of mental health using evidence based interventions within scope of professional competence.
o Effective physical health signposting/ co-working with the primary care team/ G.P
o Facilitation of safe and effective transitions of care, including: appropriate referrals into secondary treatment, primary care and other health care providers and ultimately discharge from the team caseload.
o Sharing the management of a complex team caseload and responsible for clinical supervision of other team members where identified.
o Delivering evidence based and/or psychologically informed brief interventions, in accordance with an agreed care plan, to an identified group of people experiencing mental health related difficulties within the IOM program. The post-holder is responsible for the provision of planned recovery focused care working within a defined caseload.
o The post-holder is responsible for working collaboratively with IOM partner colleagues and other Trust services to provide seamless care for service-users on defined care-pathways.

Qualifications & Training
Essential criteria
- Registered mental health Nurse on NMC Register/ Registered Occupational Therapist on HCPC register/ Registered Social Worker on Social Work England register.
- Training and experience of delivering evidence based and/or psychologically informed brief interventions such as CBT skills, EMDR, MI, SFT.
Experience
Essential criteria
- Evidence of extensive post registration experience with working with adults experiencing mental health related difficulties.
- Experience of working in community setting and making decisions autonomously
Desirable criteria
- Experience of working with the criminal justice system.
- Knowledge and awareness of the wide range of the statutory, independent and third sector service provisions for adults in the local area.
