An exciting opportunity has arisen within our Integrated Mental Health Team in Stafford. Integrated Mental Health Teams offer community based interventions for people experiencing a range of mental health conditions.  The teams have been developed in line with MPFT’s Community Mental Health Transformation Framework to offer integrated, place based community mental health services. 

We are looking for a Band 6 Community Mental Health Nurse or a Band 5 development post to join our dynamic and friendly Integrated Mental health Team based in Stafford.

The Integrated Mental Health Team is an innovative secondary care community team that is guided by collective leadership, providing evidence-based psychological and medical interventions to patients with a wide range of complex mental health needs.   This role will have a focus on mental health needs of a non-psychotic nature (i.e depression, OCD, anxiety, trauma, personality disorder etc.)

The usual hours of work are Monday to Friday 9am – 5pm. You will be expected to travel over a large geographical area and so access to suitable transport is essential. 

Main duties of the job

As part of this role, you will be expected to:

• You will, in conjunction with the multi-disciplinary team, provide nursing interventions to support the person to meet their agreed goals and coordinate and/ or participate in the review of progress towards those goals, utilising recommended outcome measures, involving the service user and any identified carer throughout the clinical pathway.
• Responsible for participating in the setting of quality standards, including the auditing, monitoring, and reviewing of practice in line with current clinical guidance practice and policy.
• Administer medication if appropriate to the role, complying with relevant Trust procedures and NMC Guidance.
• Assess patient's individual care needs, develop, implement, and evaluate programmes of care.
• Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance including pharmacological and psychologically informed interventions.
• Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.
• Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions
