Clinical Responsibilities

• Support and manage onward referrals to other professionals as required to ensure high-quality, coordinated care for children and young people.

• Undertake health assessments, screening, and redirection of referrals for children and young people aged 5 years and over.

• Use evidence-based practice to assess, plan, implement, and evaluate care for children and young people with long-term health and neurodisability conditions.

• Provide specialist clinical information and advice to support the development of Education, Health and Care Plans (EHCPs) as requested.

• Work in accordance with the NMC Code of Professional Conduct, maintaining accountability and responsibility for all aspects of professional practice.

• Demonstrate clinical expertise and sound understanding of the identification, assessment, treatment and management of the client group.

• Manage an independent caseload and work autonomously within professional boundaries.

• Act as a positive role model, ensuring that infection prevention and control policies are prioritised to maintain a safe clinical environment.

• Promote equality, diversity, and inclusion in all aspects of care and professional relationships.

• Uphold exemplary standards of behaviour, professionalism, and compassionate care in all work practices.

• Provide guidance, supervision and mentorship to junior staff and learners in line with NMC standards for mentors.

• Promote health and safety by identifying, assessing, and managing risks and ensuring compliance with Trust policies.

• Demonstrate understanding of patient pathways and care plans, reviewing and implementing changes when required to improve outcomes.

• Ensure patient outcomes are achieved in a timely manner, in line with Trust, Government and statutory policies.

• Maintain up-to-date mandatory training compliance.

• Demonstrate flexibility to support service delivery during staff absence or changing priorities.

Systems and Equipment

• Maintain accurate, legible, and timely clinical records using the current electronic patient record system.

• Manage and utilise resources effectively, including staff, equipment, and supplies, while promoting efficient use of team skills and knowledge.

• Ensure appropriate data collection and reporting systems are in place and accurately maintained.

• Ensure the safe use, cleaning, and maintenance of clinical and office equipment.

• Regularly use visual display units (VDUs) and telephones as part of daily duties.

Decisions and Judgements

• Facilitate, develop and deliver individual programmes and packages of care, ensuring high standards and continuity of care.

• Provide specialist nursing advice to the multi-disciplinary team and contribute to the delivery of clinical governance objectives.

• Promote and implement evidence-based practice to improve patient outcomes.

• Monitor and maintain standards of care through supervision, audit, and teaching.

• Reflect on own practice, identify development needs and take action to meet them.

• Participate in annual appraisal, clinical supervision, and the mandatory training programme.

• Lead and support service developments and change initiatives, encouraging innovation and adaptability within the team.

• Take case management responsibility in clinical meetings, providing clear clinical advice on assessment, risk assessment, formulation and treatment.

• Undertake supervision and appraisals for junior staff as required.

Communication and Relationships

• Establish and maintain effective communication and working relationships with children, young people, families, and professionals across health, education, and social care.

• Act as key worker when appropriate to ensure coordinated and consistent care.

• Identify and report any safeguarding concerns in accordance with Staffordshire Safeguarding Children’s Board (SSCB) guidelines and the Children Act (2004).

• Attend and contribute to core groups, case conferences, and multi-disciplinary meetings, preparing statements and reports as required.

• Support the engagement of service users and families in care planning and decision-making.

• Implement and support transition planning to adult services, ensuring a smooth and informed process for service users and families.

• Support the team in creating a positive learning environment that encourages personal and professional development.

• Communicate complex and sensitive information clearly and compassionately, both verbally and in writing.

• Prepare analytical reports and present these to internal and external agencies as required.

• Contribute to the development and implementation of health promotion and education strategies within the service.

• Identify and address barriers to effective communication within and across teams.

Physical Demands of the Job

• Travel between sites across Staffordshire as required.

• Undertake walking between offices and departments to move files or equipment.

• Carry out computer-based data collection and administrative tasks.

• Safely move and handle children in accordance with Trust policies and training.
