Older Adults at Royal Stoke promote independence and are at the forefront of the Deconditioning Campaign. We have champions on each ward and are keen to ensure our patients are engaged in diversional therapies.
We are working closely with local partners to ensure a seamless patient journey from care settings to acute beds and home again. We have implemented a Red Bag scheme, which improves communication between facilities and help towards reducing length of stay within this client group.
Within our emergency department we are proud to operate a “Frailty Front Door” service which prioritises the ‘right patient into the right place at the right time’. Following the commencement of this service our feedback has been extremely positive.
