The Primary Care Directorate is on exciting journey of expanding the services we offer.
We have an exciting opportunity for a Band 6 Practice Nurse to expand their skills in Moorcroft Surgery. This is a part time role working 28.5 hours per week.
Practice Nurse experience is essential for this role, so if you are looking for a new challenge and want to be part of a supportive MDT then this role could be for you.
Main duties of the job
You will be responsible for the assessments of care needs and the development, implementation and evaluation of programmes of care.
You will be expected to carry out all relevant forms of care.
You will be required to act in accordance with the legal requirements and statutory rules relating to Practice and all North Staffordshire Combined Healthcare
To take responsibility for patients as delegated by the nurse-in-charge.

Detailed job description and main responsibilities

- To assess patients upon initial contact, and devise an appropriate treatment or referral plan, to change dressings, remove sutures, syringe ears and administer injections.
- To perform ECG's and present results for interpretation to GP.
- To initiate and perform more complex diagnostic tests and to interpret and refer as appropriate.
- To initiate and perform venepuncture and be responsible for safe collection of specimens.
- To perform cervical smears and vaginal swabs.
- To perform health assessments and new patient interviews.
- To be responsible for the management of nurse-led clinics including travel and childhood immunisations, diabetic and asthma clinics, HRT and contraceptive counselling, Coronary Heart Disease clinics, Smoking Cessation clinics, COPD clinics.
- To contribute towards clinical targets included in QOF and QIF.
- To work within current clinical practice and guidelines including NICE.
- To comply with all policies, protocols and systems within the organisation.
- To contribute towards CQC compliance and CQC reporting systems within the practice.
- To improve patient outcomes for long term conditions by completion of standard clinical templates and working with senior clinicians to maximise patient health for example blood pressure, cholesterol, HbA1c within acceptable range.
- To prepare and assist with minor operations, including coil/implant fittings.
- To advise patients on health care, counsel and promote a healthy life-style and to screen for risk factors including cessation of smoking, diet and weight.
- To offer either a brief or intensive intervention or appropriate referral for smoking cessation support, alcohol screening and support and physical exercise and support.
- To help patients to interpret the implications of diagnosis and treatment in situations which might be contentious or sensitive.
- To observe patients' physical and mental conditions and report.
- To be responsible for maintaining accurate patient records and to ensure that all information entered onto the computer is in a retrievable form in order that the data-base continues to be established.
- To initiate any research projects which are deemed beneficial to the practice and its patients and improve the delivery of care in the practice.
- To be involved in any audit, validation and research which the practice requires.
- To be responsible for the accurate and timely claiming of items of service due to the practice.
- To evaluate the outcomes of any Health Promotion interventions.
- To be responsible for the accurate and timely claiming of vaccines dispensed by the practice.
