Job summary:

To provide a high standard of health care to patients which have been delegated to you within their own homes, residential care homes, and in clinics.

Working Pattern:

The service operates between 8am and 6pm, Monday to Sunday inclusive of Bank Holidays and you will be required to work shifts between these times.

Job responsibilities:

Key Tasks & Responsibilities:

· Phlebotomy

· Non-invasive swobs (ie MRSA)

· Observations (blood pressure, temperature etc)

· Sample collection

1. Communication

· Liaise with other health care professionals as required.

· Overcome barriers to understanding, e.g. patients’ with hearing or cognitive impairment.

· Communicate with patients/carers by exchanging factual information, giving reassurance, with tact and empathy.

To know when to escalate concerns and receive guidance to resolve these, inclusive of remote guidance and support whilst working alone.

Education and qualifications

Essential criteria

Level 1 qualification or equivalent in both Maths and English* OR undertake a 90 minute assessment and if successful you will be invited to interview
Care Certificate or commitment to undertaking and achieving the Care Certificate within initial twelve weeks

Desirable criteria

Vocational qualification at Level 2 in Health, Health and Social Care or sector related subject area

Experience and knowledge

Experience of working / communicating with the general public

Tech-enabled outcome report writing

As a Senior Assessor, you have the opportunity to focus on delivering direct care. Furthermore, if you have the relevant experience and skills, you can take on an additional responsibility. We limit this to one responsibility to allow for focused time on that topic, rather than having your time stretched too thinly. When taking on an additional responsibility, your case volume is reduced, making time available for this focused time.

For Senior Assessors, additional responsibility options include supporting more complex cases, ensuring quality assurance, providing training, facilitating family engagement, and contributing to pathway development.
