To work flexibly in a highly regarded team , who strive to provide high quality care primarily to older people, alongside their family and carers by ensuring they experience positive mental health care.  We are looking for passionate nurses who will focus on providing support and high-quality nursing care for clients and carers, targeting and prioritising people with severe mental health problems to join the Mid  Community Mental Health Team for Older People (CMHT OP)

We are based in the heart of Epsom , the CMHT OP service offers assessment, diagnosis, treatment and support for people over 65 who present with functional mental health illnesses; deterioration in cognition and/or dementia. We work closely with other services, such as social care and also with the voluntary sector, particularly the Alzheimer’s Society, to provide positive support for people using services and their carers, ensuring effective forward signposting where appropriate

To act as key worker / care co-ordinator for named individuals, within the Care Programme Approach process, including Single Assessment. To provide high standards of care, which are delivered in a professional manner. .To ensure all records and reports are kept up-to-date, and up to the standard expected of this grade of nurse, in accordance with the Caldicott Report. Be responsible for the continuous overall management of a defined caseload. To receive and give clinical and management supervision / guidance. To act as a positive role model for junior staff, assisting / guiding them in their duties. To provide information to the Line Manager, if any incidents that may affect the standing or running of the service.  To develop close working relationships with voluntary / statutory services, in particular Social Services and members of the Primary Care Team, and participate in the system of integrated team working. To educate and train junior staff / student nurses and other professionals around competencies of a Community Mental Health Nurse. To participate in the Duty Rota to manage new referrals

As this role is based within a community setting we do require that you must be able to travel between multiple sites and trust locations within the set hours of your working day.

This is a role suitable for an NMC registered Community Psychiatric Nurse who is passionate about delivering excellence in dementia care in care homes.

Person specification

Essential

Essential criteria
  • RMN qualification
  • One year post-registration experience Previous community experience
  • Have a good understanding of the roles and functions of the CMHTOPs,
  • Demonstrate an awareness of multi-agency working and integrated care teams
Desirable criteria
  • Mentorship course or equivalent supervisory training CPN course Other appropriate qualifications

Clinical

Essential criteria
  • Ability to work alongside other professionals/engage in integrated working to deliver high quality care and services
  • Understanding of the Care Programme Approach and single assessment process
  • Demonstrate an awareness of the importance of good relationships with people using services, family/paid carers and other colleagues
  • Knowledge and use of the Mental Health Act and other frameworks such as the Mental Capacity Act and Deprivation of Liberty Safeguards
  • Knowledge of key documentation and policies related to the evidence-base for dementia care and the post holder’s professional specialty
Desirable criteria
  • Have a working knowledge of CPA areas A willingness to professionally develop
  • Evidence of completing education programmes/specialist training
