Job Overview

To assess, plan, implement and evaluate the physiotherapy needs of people with learning disabilities who use our services.

To supervise and teach unqualified staff including students.

To foster a culture that encourages and enables individuals to have an active role in identifying, planning, and meeting their individual needs.

Main duties of the job

To carry out comprehensive assessments, plan implement and evaluate delivery of physiotherapy interventions as part of a holistic approach working in a variety of settings.

 

To demonstrate specialist physiotherapy assessments such as mobility, falls, tone management – using a variety of modalities.

 

To do full postural assessments and lead on the 24-hour postural management programmes.  Additional internal and external training will be provided to ensure proficiency in this.

 

To work as part of a multi-professional team to help holistically treat the service user.

·         To actively involve service users and their carers (where required) in developing and agreeing their own care plan, to act as an advocate and ensure that there is proper regard for their dignity, choice, self-esteem, racial, cultural, sexual, and religious needs and preferences.

 

 To practice within the Positive Behaviour Support framework.

 

To undertake comprehensive risk assessments and develop effective risk management plans.

 

To work in partnership with acute and primary care services and other healthcare providers.

 

 To establish and maintain effective relationships with colleagues, other agencies, people who use services and their families.

Detailed job description and main responsibilities

To use a person-centred approach and current evidence base to carry out comprehensive assessments, plan, implement, evaluate, and document interventions with the support of senior colleague.

 

To have professional expertise and a wide base of clinical skills in areas including falls prevention and management relating to individual patients over the age of 65. This will include working as an autonomous clinician, with the responsibility for the effective management of falls within an identified case load.

 

To demonstrate specialist expertise and physiotherapy skills in different areas and to have an extensive range of clinical skills, knowledge, and experience appropriate to the needs of individual patients over the age of 65 years presenting with mental health needs.

 

To show a sound understanding of legal and ethical issues in advance care planning.

 

To liaise with other necessary agencies to establish robust communication networks with carers, hospitals, other health workers and agencies to facilitate an integral approach to patient care.  This includes participation in multi-professional meetings, case conferences and other team meeting where they may be the sole representatives of their professional group.

 

To use a range of verbal and non-verbal communication tools to communicate effectively with the people who use our services to progress rehabilitation and treatment programmes.  This will include people who may have difficulties in understanding or communicating.  For example, service users may be dysphasic, depressed, deaf, blind or who may be unable to accept diagnosis.

 

To undertake comprehensive risk assessments and develop effective risk management plans.

 

To provide spontaneous and planned advice, teaching and instruction to relatives, carers and other professionals, to promote understanding of the aims of physiotherapy and to ensure a consistent approach of care.

 

To be responsible for the safe and competent use of physiotherapeutic equipment such as walking aids and any other rehabilitative equipment or assistive device ensuring service users, carers and relevant team members attain competency in the safe use of all equipment.

 

To demonstrate a high standard of record keeping and documentation with adherence to trust policy and an understanding of professional guidelines and application of confidentiality.

 

To input into the development of evidence based, effective and appropriate clinical service across the inpatient wards.

 

To undertake audit, evaluating the effectiveness of interventions in line with national and local recommendations as directed by Senior Colleagues.

Essential criteria
  • Degree In Physiotherapy or equivalent
  • HCPC registered

Experience

Essential criteria
  • Working in an inpatient ward
  • Experience completing mobility assessments.
  • Experience completing initial assessments and risk assessments
  • Excellent verbal, non-verbal, and written communication skills, along with clear and logical presentation skills
  • Knowledge of clinical/risk assessment and understanding of Information Governance principles
Desirable criteria
  • Experience in SystmOne
  • Experience working with older adults
