Main duties of the job
- To provide high quality care to clients under the indirect supervision of Team manager within the community bridging service.
- To support clients with activities of daily living in a clients home
- Ensure high standards are maintained in a safe environment, which promotes equality and sensitivity for all individuals
- To participate in continuous professional development and other team activities
- Carry out a range of duties within agreed competency skill set e.g. meal prep
- To ensure that all patients on the caseload are safe.

Qualifications
Essential criteria
- Level 2 Apprenticeship in Care (or relevant subject) or a minimum of 1yr experience in health and social care setting
- GCSE maths and English (or equivalent)
Desirable criteria
- Level 3 Apprenticeship in Care (or relevant subject)
- Care certificate
Experience
Essential criteria
- Previous experience in care eg as HCA
- Safe moving and handling experience
Desirable criteria
- Previous community experience
- Complex moving and handling experience
Skills & Knowledge
Essential criteria
- Moving and handling of clients
- Personal care to clients in a person-centred manner
- Follow clear instructions and feedback to senior staff
- Work safely with minimal supervision
Desirable criteria
- Knowledge of some medical conditions / exercise therapy
- Good interpersonal and communication skills both verbal and written
