Job overview

We are keen to recruit a Band 4 Associate Practitioner to provide specialist input to the Care Home Pathway and the Post Diagnostic Clinic within the Surrey Heath CMHTOP. 

You will be working alongside colleagues from a range of professional backgrounds (psychology, occupational therapy, nursing and associate nurse practitioners) to provide specialist advice and consultation to people with dementia living in care homes within the local area.  You will be assisting people who are displaying behaviours that challenge themselves and/or others by drawing on ideas from a Positive Behaviour Support framework. 

You will also be supporting the CMHTOP in post diagnostic work and running/facilitating groups for patients with functional and or organic presentations.

Main duties of the job

To succeed in this role, you will be supported by a range of specialist clinicians and there are regular opportunities for training, CPD and supervision in relation to the care home work.  This includes quarterly CPD events scheduled for all staff working into the Care Home Pathway, where you will receive specialist training and share ideas with other colleagues working in a similar role. 

We are keen to meet individuals with a passion for learning new skills, particularly in the field of behaviour that challenges in dementia care.  Skill development will be supported through training on Positive Behaviour Support and psychosocial approaches to supporting people showing distressed behaviour. 

The post will be based at Theta, Frimley, but travel will be expected across the SH CMHTOP region.   

As this role is based within a community setting we do require that you must be able to travel between multiple sites and trust locations within the set hours of your working day. There is also an expectation that you will deliver personal care for people with dementia and experience of working in care settings would be an advantage for this position.

Qualifications

Essential criteria
  • Nursing Associate Foundation Degree
  • BSc Psychology degree
Desirable criteria
  • Evidence of continuous professional development since graduation
  • Additional skills related to delivering care to people with mental health needs
  • Additional skills related to delivering care to people with dementia
  • Additional skills related to delivering care to people with dementia in care homes

Experience

Essential criteria
  • Experience of working with individuals experiencing mental health difficulties
  • Experience/Knowledge of working with people with dementia in care homes
  • Driving licence
Desirable criteria
  • 2 years’ experience working in a role delivering care in a mental health setting
