Job overview
The post holder will work within the Children and Family Health One Stop referral centre (no face to face patient contact) as part of a skill-mixed team. Main duties will include the digital screening of referrals to ensure prioritization, safety, accuracy. This includes making clinical decisions about the eligibility for access to services and applying a “no wrong door” approach. This is with a focus on more complicated referrals which may include complex health needs or safeguarding elements.
This role is suitable for someone who has clinical expertise and experience in children’s health and development. They must be confident to lead and motivate the staff team in pursuing an excellent quality of care and they will support clinical and day-to-day leadership of the team
Main duties of the job
Making clinical decisions on referrals about the eligibility for access to services.
Use Clinical expertise to triage referrals to the appropriate services
Be able to liaise with services regarding referrals.
Have phone conversations with parents, referrers etc and aim to assist to the best of your ability.
Support the team in service development and transformation.
Be aware of the continuous service changes and be able to apply these daily

Qualifications
Essential criteria
- A qualified Nurse or Allied Health Professional e.g. RCN, SCPHN, SLT, OT, Physio or equivalent
- clinical registration with a recognised professional body (e.g. NMC, HCPC)
Experience
Essential criteria
- minimum of 3 years post registration experience
Applicant requirements
You must have appropriate UK professional registration.
The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
