Job overview

The post holder will provide specialist dementia expertise to the multidisciplinary team to ensure that patient care is safe and of high quality. To work collaboratively across professional and organisational boundaries to provide a holistic and comprehensive assessment of patient need and ensure appropriate care and timely and safe discharge. To provide education and training to members of the multidisciplinary team to raise awareness of dementia, delirium and person-centred approaches to care. The post holder will work closely with the lead nurse to ensure the co-ordination and delivery of dementia care services in accordance with the National Dementia Strategy and Trust objectives. 

This post includes caring for patients at UHCW and Rugby St. Cross Hospitals who present with dementia and/ or delirium, dependent on service need cross-site cover may be required in the role.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP. 

Main duties of the job

Working as an autonomous practitioner within the multidisciplinary team (MDT), the post holder will undertake holistic assessments, develop comprehensive care plans, and provide specialist advice on the management of behavioural and psychological symptoms of dementia and delirium, including complex and challenging presentations. Close collaboration across professional and organisational boundaries to ensure appropriate care, effective discharge planning, and seamless transitions between acute and community services.

Work within the framework of the Mental Capacity Act, supporting capacity assessments and best interests decision-making while upholding patient rights. Act as a clinical role model in accordance with the Nursing and Midwifery Council (NMC) Code and provide visible clinical leadership to promote excellence in dementia care.

Education and workforce development are central to the role. The post holder will deliver training, facilitate learning opportunities, and provide bedside coaching to enhance staff knowledge of dementia, delirium and person-centred care approaches. 

Promote evidence-based practice, contribute to research and audit activity, support policy and guideline development, and participate in quality assurance processes. Act as a resource for staff, patients and carers, support investigations following incidents or complaints, and contribute to the ongoing development of dementia services in line with the National Dementia Strategy and Trust objectives.

Detailed job description and main responsibilities

  • Act as an autonomous practitioner to provide assessment, advice and liaison within University Hospital Coventry and Warwickshire NHS Trust hospitals supporting people with suspected or diagnosed dementia, delirium, who have been admitted to hospital for a primary physical health reason.
  • Liaise with colleagues and provide education, support and advice to staff within the hospital setting on a range of subjects aimed at improving their understanding and management of dementia, delirium and any associated behavioural and psychological symptoms.
  • Provide specialist advice on care planning and interventions in relation to individual patient care to staff as well as informal carers.
  • Provide advice and support on management in challenging clinical situations and develop specialised care plans e.g. Assessment and management of patients with behaviours which challenge.
  • Work strictly within the framework of the Mental Capacity Act to ensure individual’s rights to make their own decisions are always respected and to fully support those individuals who are deemed to lack capacity by contributing to any best interests plans.
  • Act as a clinical role model for effective clinical leadership enabling and facilitating others to take initiatives forward
  • Always maintain a professional attitude, developing the role of the nurse in the best interests of the patient and profession in accordance with the Nursing and midwifery (NMC) Code of Professional Conduct
