If so, then we are keen to hear from you. We are looking to expand the leadership team and welcome you to apply for the position of Community Nursing Sister/Charge Nurse. You will be supporting the staff within the team, have the privilege of delivering high quality nursing care to our patients and supporting those around them to ensure we keep them in their own homes for longer managing either their long or short term conditions.

A full UK driving license is essential for this post. Please note that an international driving license is not accepted.

Secondment will be considered for internal candidates – If you are successful after interview, you must obtain approval from your current line manager to accept the offer.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP. 

Main duties of the job

Responsible for the coordination and provision of planned and unplanned care to patients and their carers in a defined geographical area.

  • Triage and accept referrals direct from other disciplines, patients and carers and refer to other agencies where appropriate.
  • Case manage packages of care and work within the framework of the community nursing team by providing holistic care
  • Undertake comprehensive risk assessments associated with the care of patients in order to ensure nurses’ and carers’ safety.
  • Act as a role model for other staff and students demonstrating high standards of practice and professional conduct.

Detailed job description and main responsibilities

  • Support the Senior Management Team in ensuring all obligations of service delivery are met
  • Ensure practice is evidence based through undertaking audit and critically appraising literature to achieve a high standard of care.
  • Involve patients and carers in planning and delivery of care
  • To competently perform a range of clinical skills related to the post
  • Ability to work without direct supervision and to solve problems
  • Work on a rota basis to meet the service requirements
  • Maintain a professional image
  • Attend professional meetings and case conferences as deemed necessary

The group is currently trialling a new Integrated Neighbourhood Team (INT) model following a collaborative event with internal and external stakeholders. This pilot aims to explore team structures and assess the impact on patient experience. While no formal redesign has taken place yet, the outcomes of this trial may inform future service delivery and team configuration across the group.

If you are successful in your application, please be aware that your team structure and line management may be subject to change in the future as part of this evolving model. However, any formal changes will be preceded by appropriate consultation in line with organisational policy.
